    What You Will Do:

  • Under the supervision of the Senior Director of Behavioral Health, provides case management services to assist with discharge planning.

  • Serves as the primary liaison between inpatient and outpatient Behavioral Health services to ensure continuity of care.

  • Is cross-trained to assist with other psychosocial services on the unit as needed.

  • Renders all services within department guidelines and in compliance with rules/standards set forth by Medicaid/Medicare, HFAP, OhioMHAS, and CARF.

  • Demonstrates a commitment to the provision of quality care through engagement in professional growth and agency-sponsored learning opportunities; utilizes best practices as appropriate to the conditions being treated; and provides all services in a manner that instills hope and meets patients where they are at in terms of their readiness for change. Uses best practices for patient engagement.

  • Promotes patient recovery through active and collaborative patient involvement in the development of discharge plans. Involves natural support persons, self-help resources, and other community integration activities as critical components to long-term recovery.

  • Maintains current knowledge of outpatient and community-based resources through active involvement with outpatient case management teams including attendance at outpatient staff meetings and other pertinent trainings as needed.

  • Understands the guidelines and resources pertaining to guardianships, group home placements and other residential settings.

  • Alerts outpatient teams to discharge planning needs including referring those in need of intensive outpatient services such as Day Treatment and/or Substance Abuse Intensive Outpatient treatment.

  • Provides timely and accurate documentation in both inpatient and outpatient electronic medical records in compliance with departmental guidelines.

  • Ensures all electronic forms of communication, that include PHI, are compliant with HIPAA and 42 CFR laws regarding confidentiality.

  • Positively contributes to patient, referral sources, and employee satisfaction. Is a team player who is flexible, willing to assist team members, and positively influences team morale. Actively participates in and follows through with strategic planning, performance improvement and quality initiatives.

    What You Will Need:

  • Associate’s degree or higher in social services related field required. A minimum of two years full-time equivalent experience working with persons with mental health or substance use in a community-based, publicly funded system is required.

  • Heart Saver, Aegis Crisis Prevention, and First Aid certification within 6 months of hire.

  • Ability to be mobile and possess a valid State of Ohio motor vehicle operator’s license and be insurable under Firelands’ auto policy. Employee must provide proof of personal automobile liability insurance coverage upon request.

  • Successful completion of a 90-day probationary period.
