Community Living Case Lead (DSP/Supported and Independent Living, Full-Time or Part-Time)

Toolworks' Community Living Department is currently looking for dedicated, creative individuals to join our team! We are accepting applications for the following positions:

  • Case Lead - Independent Living Skills (ILS) and Supported Living Skills (SLS)in theSan Francisco Bay Area (San Francisco, Alameda, and San Mateo counties)
    • Compensation: $23.25/hour.
    • Primary Role:To provide in-home support, case management, instruction and training to clients in all areas of daily living, including, but not limited to meal preparation, shopping, hygiene, personal care, budgeting, medical appointments, travel, community access, safety awareness, and maintaining a clean and healthy living environment
    • Work Schedule:Case Leads work directly with an assigned case load of individuals receiving Independent Living (ILS) or Supported Living Services (SLS) to provide daily support and case management functions. Typical Case Lead schedules are Sunday-Thursday or Tuesday-Saturday. Daily schedule is dependent on case load. Schedules can be tailored to meet individual employee needs and interests.

``` - Case Lead Floater- Weekends, On-Call Support - Compensation: Case Lead Floaters will be paid at their hourly rate, $23.25/hr, for all shifts they personally cover during the rotation (sleeping overnight shifts are paid at the prevailing local minimum wage). A stipend of $500is paid to Case Lead Floaters able to monitor and respond to calls during the full rotation: Fridays at 3pm to Monday at 9am. For Case Lead Floaters unable to make a commitment to the full rotation, a stipend of $75 shall be paid for daily rotation. - Primary Role:Case Lead Floaters are responsible for assisting the Community Living management team in providing the day-to-day support of our clients receiving Supported Living Services. A Case Lead Floater's role is to ensure client shifts are filled and Community Living support staff are trained in their duties. - Work Schedule:Case Lead Floaters will be on-call Friday at 3pm to Monday at 9am. During this time, the Case Lead Floater will be responsible for monitoring calls coming into the 24/7 Emergency Response System (ERS) via an iPhone. The nature of calls varies from non-life-threatening issues/concerns, staffing questions, injury reports and staff call-outs or late arrivals. - There is potential for the Case Lead to be assigned a guaranteed amount of shift work hours during the week for those who desire to be in a benefitted status. - The Case Lead Floater will provide coverage for any open shifts and for staff absences/vacations. The shifts worked would vary according to the support needs of our clients: mornings, afternoons, evenings, overnights may be included. - Volume of calls varies weekend to weekend. The Case Lead Floater will be made aware on Friday before 3pm if there are known open shifts that need coverage provided by the Floater. - Case Lead Floater rotation schedule may change as program needs change.

Duties and Responsibilities

Client Support Functions

  • Develop written plans
  • Develop work/volunteer opportunities
  • Provide benefits resources as needed
  • Counsel using a wellness and recovery model
  • Teach client rights and help develop self-advocacy skills
  • Assist clients in developing natural networks of support
  • Develop on-site activities that foster community building and self-sufficiency of resident populations
  • Provide linkages in the community for needed services; provide on-going case management and service coordination

Administrative Functions

  • Responsible for completing all required documentation, including monthly billing reports, SIR's, case notes and additional case records

Full paid training provided.

Education and Experience Requirement

  • B.A. n Special Education, Rehabilitation Counseling or related field preferred.
  • Minimum of two years' experience working with adults with disabilities required, preferably in a community setting.

Skills requirement

American Sign Language, Spanish or Cantonese skills preferred. Some level of fluency for certain target populations may be required.

Fluent report writing skills.

Willingness to work flexible hours to meet client needs.
