Robert Half Case Manager in San Francisco, California

Description We are offering a short term contract employment opportunity for a Case Manager in San Francisco, California. This role is vital in our team as it involves managing cases of formerly homeless individuals, many of whom struggle with mental health conditions, substance abuse issues, and/or medical issues.

Responsibilities

• Provide comprehensive case management services to clients, focusing on housing retention and improving life quality

• Conduct initial and ongoing assessments of clients' needs, and provide follow-up visits as required

• Build rapport with clients and engage them frequently with effective intervention strategies to help them retain their housing

• Assist clients in maintaining their benefits and ensuring regular rent payments

• Address unit habitability issues and accompany hotel managers on monthly pest control and unit inspection visits

• Refer clients to job placement programs as needed and support them in connecting with these services

• Foster an active and supportive community by hosting regular events, tenant groups, and social activities

• Maintain detailed and current documentation and database records of client interactions and services provided

• Respond to client-related crises, offering de-escalation intervention and support

• Collaborate with other departments and external providers to advocate for and support clients

• Attend all scheduled meetings and participate as requested. Requirements • Possess strong skills in Administrative Assistance, adept at managing schedules, coordinating meetings, and handling correspondence

• Proven proficiency in Data Entry, with the ability to quickly and accurately input, manage, and retrieve data

• Demonstrated experience in Filing, capable of creating, maintaining, and updating efficient filing systems

• Ability to manage multiple cases simultaneously, prioritizing tasks based on urgency and importance

• Excellent interpersonal and communication skills, capable of liaising with clients, families, and other professionals

• Strong problem-solving abilities, with a knack for developing effective solutions to case-related issues

• Familiarity with case management software and other related technologies

• Bachelor's degree in social work, health care, or a related field

• Minimum of 2 years' experience in a case management role or similar position

• Ability to maintain confidentiality and adhere to ethical standards

• Understanding of relevant laws, regulations, and protocols related to case management

• Ability to work independently and as part of a team, demonstrating strong collaborative skills

• Excellent organizational skills and attention to detail, ensuring accurate and timely case handling
