We are looking for an energetic person comfortable with multitasking. Our unit is busy and requires attention to the myriad of things that happen on a daily basis. If you are looking for a challenge with great rewards you should apply. Some of your duties are listed below.

· Providing/overseeing psychosocial evaluation/consultation/referral and triage of assigned patients.

· Conducting clinical assessments of patient/family/total situation to establish intervention strategies.

· Developing resources such as educational programs for community agencies or hospital or ambulatory personnel.

· Facilitating patient care conferences with other team members to review patient's goals of care, provide psychosocial support, assist with advance care planning and ensure proper referrals are made to community resources.

· Providing and/or coordinating in-service education and training for staff.

Required Qualifications:

· Master’s degree in Social Work (MSW)

· LCSW

· BLS for Healthcare Provider American Heart Association

· Working knowledge of the laws, regulations, policies and appeal procedures as relative to the Federal, State and other reimbursement programs

· Recent experience in acute care hospital as a Medical Social Worker, including knowledge and proficiency of discharge planning

· Must be able to display good working relationships with patients, families, medical and nursing personnel, and all others employed at the hospital

Preferred Qualifications:

• Proficiency in Microsoft Office including but not limited to Outlook, Excel Epic, ECIN.
• An ability to speak another language is desirable.
