Hourly Pay Range (Minimum - Midpoint - Maximum):

$50.611 - $65.305 - $79.999

The stated pay scale reflects the range that Sharp reasonably expects to pay for this position.  The actual pay rate and pay grade for this position will be dependent on a variety of factors, including an applicant’s years of experience, unique skills and abilities, education, alignment with similar internal candidates, marketplace factors, other requirements for the position, and employer business practices.

What You Will Do

This position is responsible for assuring patient safety, providing excellent evidence-based quality of services, and assuring timely and accurate quality review in compliance with the hospice and palliative care quality/safety plan. The Senior Specialist Performance Improvement-RN participates in the development and implementation of Sharp HospiceCare and Palliative Care's Quality and Safety Plan, including performance targets. This position is responsible for data analysis, assisting with identifying real and potential quality issues, and reporting to leadership teams/committees to drive performance excellence as well as meet and insure compliance with regulatory requirements, laws, and hospice and palliative care policies and procedures. Assists with operational planning and provides daily work direction to support staff to achieve objectives of the quality/safety plan.

Required Qualifications

  • Bachelor's Degree in Nursing

  • 3 Years Clinical RN experience

  • California Registered Nurse (RN) - CA Board of Registered Nursing -REQUIRED

  • Driver's License - CA Department of Motor Vehicles -REQUIRED

Preferred Qualifications

  • 1 Year Hospice experience.

  • 2 Years Previous case review and experience in performance improvement (PI).

  • 3 Years Hospital acute care experience

  • Certified Professional in Healthcare Quality (CPHQ) - National Association for Healthcare Quality -PREFERRED

Other Qualification Requirements

  • Utilizes reliable transportation and possesses adequate personal insurance coverage. Demonstrates clean driving record in accordance with requirements of the employer DMV pull notice program and Sharp HealthCare Driver Guidelines.

Essential Functions

  • Analyzes and Leads Improvement in Clinical Operations PerformanceUtilizes independent judgment, training and clinical experience to recognize variances in quality/patient safety and opportunities for performance improvement. Participates in implementation of practices and procedures to improve quality of patient care.Identifies statistical trends in data through utilization of independent judgment, training and clinical experience in analysis of case reviews, system reviews, and/or of clinical data.Conducts accurate, comprehensive and systematic reviews of clinician performance and ongoing measurement of operational process and outcome data through utilization of independent judgment, training and clinical experience.Determines and implements appropriate performance improvement process (such as 6-Sigma/DMAIC and Failure Mode Effects Analysis) to drive improvements in entity, department, and physician performance.Expert in utilization of performance improvement tools.Participate in Root Cause Analysis and/or facilitate performance improvement projects.Takes initiative in department projects.Facilitates successful performance improvement by working collaboratively with department leaders, colleagues, and customers to drive improved patient care results and provide patient safety and excellent clinical patient care.Manages data for quality indicators consistent with department norms/standards.Provides education and technical support specifically for the development, implementation, and maintenance of PI activities.

  • Department Performance ImprovementWorks collaboratively with leadership to set and achieve agency, department, and staff goals.Actively participates in departmental meetings.Assists other department staff with prioritization of workload.Completes other assigned projects including dealing with unanticipated urgent assignments.Maintains constructive and collegial relations and collaborates with leadership, risk management, and clinical/ancillary/support service staff in cross-functional process improvement, collaborative teamwork, open communication, and problem solving.

  • LeadershipUtilizes appropriate communication (verbal and written) and presentation skills.Provides education to clinicians to promote/improve patient safety and quality.Leads and manages work teams based on organization goals and performance improvement needs.Manages follow-up with teams/work groups to ensure projects are completed and ongoing data collection is managed and documented as needed.Provides report-out on team process and outcomes to leadership.Facilitates multi-disciplinary PI teams to address and resolve identified quality issues and improve performance.Assumes responsibilities for own professional development by attending courses and at the request of leadership, seek continuing education to enhance job function and stay abreast of regulatory changes.

  • Organizational Performance ImprovementWorks collaboratively with Manager of Quality and Performance Improvement to identify quality and patient safety issues and drive organizational performance improvement.Takes initiative to identify and improve internal department services and effectiveness including creating improvements in data processes.Leads efforts of performance improvement to enhance department and organizational effectiveness, including recommendation of appropriate coaching and/or corrective action to facilitate performance improvement.Effectively uses ongoing monitoring techniques including corrective action process, criteria development, and statistical analysis to sustain improved processes.

  • Quality Improvement and Patient SafetyManages quality and patient safety issues in the absence of the manager, including follow through on time critical reportable events.Works collaboratively to problem-solve staff issues with manager and leadership.

  • Regulatory ComplianceMaintain regulatory compliance with licensure, Medicare Conditions of Participation, and The Joint Commission.Participate in the review and revision of policies and procedures to meet regulatory requirements and department needs.Participate in the development of and coordinate the implementation of educational programs, including orientation, to address staff members needs and to meet regulatory requirements.Prepare reports for the Quality/Safety Council, as assigned.

Knowledge, Skills, and Abilities

  • Intermediate computer skills required including working knowledge of PowerPoint, spreadsheets, statistics, and clinical information systems.

  • Current knowledge of Joint Commission, State, and Federal regulatory agency requirements, standards and regulations, particularlythose related to Performance Improvement required.

  • Maintains confidentiality of quality improvement department information. Refer to quality confidentiality agreement.
