Rady Children's Hospital San Diego Lead Care Manager - Managed Care Clinical Operations in San Diego, California

Due to business needs of the department, this position requires proficiency in English and Spanish.

JOB SUMMARY:

The Lead Care Manager (LCM) operates a as a member of the patient's Enhanced Care Management (ECM) team and is responsible for coordinating all aspects of ECM and any community supports. The LCM is the primary source of contact for the patient and the family for ECM and may act as a Liaison or coordinator for PCP's, Specialists, Behavioral Health providers, community agencies or community service providers that are engaged in care coordination activities. The LCM will enroll patients in ECM, perform screenings, reinforce teaching, make referrals, and provide support to implement the Care Plan developed by the Care Management Team. The LCM assists the patients/families to develop strategies to improve health and well-being, navigate and access community services and other resources. This position involves community outreach, such as home visits required.

MINIMUM QUALIFICATIONS:

H.S. Diploma,GED,or Equivalent

CA Driver’s License

Proof of Auto Insurance

Basic Computer Skills

Good communication skills, including listening well, and using language appropriately

Ability and willingness to provide emotional support, encouragement and motivation to patients

PREFERRED QUALIFICATIONS:

Associate’s Degree

2 Years of Experience

Proficient in Healthcare Quality

Successful completion of a Community Health Worker training program

One year exp working in a psych program, schools, residential day treatment center. Time spent in mental health internships would be considered

Bilingual English/Spanish

Experience working in a multi-cultural setting

Experience working in a community-based setting for at least 1 to 2 years

Knowledge of some medical terminology preferred

The current salary range for this position is $28.09 to $38.62
