Description

Klarus Home Healthis looking for full time, part time or PRN / Per DiemLicensed Vocational Nurses (LPN / LVN)to join our dedicated team to provide community-based care for our patients currently receiving home health services in theSan Antonio, TX metro area.

The primary function of the LPN / LVN is to provide nursing care, under the supervision of a registered nurse, for patients of all ages in their place of residence. The LPN / LVN will coordinate care with the interdisciplinary group, patient/family, and referring agency to ensure the best patient outcomes.

Requirements

  • Graduate of an accredited School of Vocational/Practical Nursing.

  • Current State license as a Licensed Vocational/Practical Nurse, or a multi-state license issued by a state member of the National Nurse Licensure Compact (NLC) if practicing in a state member of the NLC.

  • Two years’ experience as a Licensed Vocational/Practical Nurse in a clinical care setting. Home health experience highly preferred.

  • EMR system: HCHB experience highly preferred

  • Proficiency in nursing skills as defined by generally accepted standards of practice. Good interpersonal skills.

  • Reliable transportation and valid, current auto liability insurance.

  • Provide nursing care to patients of all ages in their homes under the supervision of a registered nurse.

  • Coordinate care with the interdisciplinary group, patient/family, and referring agency.

  • Assist in the development and implementation of patient care plans.

  • Monitor patient health and report any changes to the supervising RN.

  • Administer medications and treatments as prescribed.

  • Educate patients and their families on health care needs, conditions, and options.

  • Maintain accurate and timely documentation of patient care and progress.

  • Work in patients' homes in various conditions.

  • Ability to work a flexible schedule and travel locally.

Physical and Mental Effort:

  • Prolonged standing and walking required.

  • Ability to lift up to 50 lbs and move patients.

  • Requires working under some stressful conditions to meet deadlines and patient needs.

  • Ability to make quick decisions and acquire necessary resources.

  • Must meet patient/family individualized psychosocial needs.

Salary Description

$22.00 - $35.00 per hour
