Job Description
The Communication Assessment Response and Evaluation (CARE) Coordinator reports to the Director of Student Rights and Responsibilities and is responsible for case management and coordination of needs of students who have health, psychological, economic, family relationships, and/or social adjustment-related problems through a variety of interventions, referrals and follow up services. The Coordinator provides consultation to campus stakeholders to minimize academic disruptions and help resolve problems affecting students in distress.
Salary: $46,000 annually
Responsibilities:
Address basic needs issues and wrap-around services for all students to include, but not limited to, addressing food and housing insecurities and financial, childcare, academic needs, and behavioral concerns.
Contact referred students, schedule meetings, assess current needs, and connect to resources.
Maintain confidential student records.
Complete documentation and maintain record keeping of cases in Maxient case management system.
Manage and make appropriate referrals to on-campus resources and community-based resources.
Coordinate and follow up with students (email, virtual meetings, in-person meetings) regarding referrals and progress made on action plans.
Document progress and close cases within prescribed timeframe.
Provide outreach and education to campus community.
Provide support and outreach to staff and faculty when distressed students are identified, and support is needed.
Participate in outreach and tabling events.
Required Education and Experience:
Bachelor’s degree or higher in counseling, psychology, social work, or related field.
Two (2) years of related experience.
Preferred Education and Experience:
Master’s degree in counseling, Psychology, Social Work, Student Affairs/Higher Education Administration or closely related field.
Three (3) years of related experience.
Experience working in a college or university setting in the area of case management.
Experience using a case management database management system.
Knowledge, Skills and Abilities:
Strong Interpersonal and presentation skills.
Excellent verbal and written communication skills to be able to interact effectively with students, faculty/staff, community providers and parents.
Ability in managing difficult and complex situations.
Excellent attention to details.
Knowledgeable about and skilled in working with a variety of mental and physical health providers and community agencies, crisis management/prevention, educational outreach to students, staff, faculty, and parents.
Ability to work cooperatively with others within a multidisciplinary team environment.
Evidence of effective collaborative relationships with students, faculty and/or staff.
Ability to develop and maintain case management files, referral resources and community services database.
Ability to maintain confidentiality based on relevant laws, policies and/or procedures.
Ability to work some evenings and weekends.
