As an Advanced Clinical Pharmacist, you will demonstrate proficiency in all pharmacist functions by attending and actively participating in daily multidisciplinary and bedside rounds, completing drug utilization review to promote rational drug therapy, leading and developing clinical services, mentoring and educating staff to ensure accurate and appropriate documentation of clinical activities. You will conduct drug regimen reviews and respond to specific polypharmacy, pharmacokinetic and medication/MAR reconciliation consults. You will recommend and monitor drug therapy as assigned by the medical staff. Utilizes expertise to train, mentor and supervise PharmD candidates, pharmacy residents, and technicians. Train and participate in facility wide committees and training programs.

Job Summary and Qualifications

As an Advanced Clinical Pharmacist, you will be responsible for participating and leading pharmacy clinical or operational programs. This includes monitoring patient medication regimens and providing appropriate interventions.

What qualifications you will need:

1) Mandatory: Doctor of Pharmacy or Bachelor of Science in Pharmacy

2) Mandatory: Minimum of 1 year of hospital clinical pharmacy experience or at least 2 years of general hospital pharmacy experience required; preferably within area of focus.

3) Mandatory: Completion of hospital PGY1 / pharmacy general practice residency or fellowship

4) Mandatory: BCPS certification or other Board of Pharmacy Specialty (BPS) certification

5) Mandatory: Current Registered Pharmacist licensed by the Texas State Board of Pharmacy or Licensed pharmacist in another state: Provide proof of the Authorization to Test (ATT) from the Texas State Board of Pharmacy.

6) Mandatory: IV certification in the state of Texas

It is home to the Texas Transplant Institute and is the largest living donor kidney transplant program in the US.
