Additional Information About the Role
Case Coordinator RN
Works hours are 0800-1630, Monday through Friday, with a weekend shift every 4-8 weeks as needed based on needs of department.
-Cover UM assignments when there are staffing gaps due to scheduled vacations or staff outages
-Trouble shoot issues
-Help onboard and train new hires
-Assess learning opportunities among the staff and conduct individual and/or team education in meetings
-Support leadership with implementing new best practices and existing workflows

Preferred Qualifications
Role Purpose
Serves as a clinical resource facilitator to the care coordination department with key focus areas that may include: level of care/medical necessity, progression of care and care transition processes, outlier management and other responsibilities in care coordination or utilization management. Works with department management to ensure designated aspects of care coordination are functioning in highest quality. This role will serve as a subject matter expert to team members involved with care coordination.
Responsibilities
Provides support to the case management program.
Coordinates orientation for new employees.
Conducts ongoing audits to ensure staff competency.
Coordinates special projects.

Education
- Nursing Diploma/Associate's - Nursing
