Telehealth Pharmacist

Job Description

Provide high touch tele-health services which include Medication Therapy Management (MTM), basic medication education, referrals to support services, etc.

Medication therapy managment – 60%

• Counseling participants on the importance of medication adherence (alerting participants to missed dosages and refills)

• Providing medication education

• Providing self-care education for specific chronic conditions

• Contacting physicians to schedule diagnostic testing

• Contacting physicians to make medication therapy recommendations

• Connecting with and supporting Community Health Workers advocating for the participant.

• Connecting participants with our community-based resources as needed.

• Assessing a participant’s health status

Documentation – 20%

• First name, Last Name, and Date of Birth

• Patient Signature or retrievable electronic verification of services received

• Date the service was provided (month/day/year)

Patient Coordination – 20%

• Coordinating a day/time to conduct MTM-related services to patients

• Coordinating patient signatures directly after service

• Coordinate and communicate with In-Store pharmacy staff about patient related topics

Pay and Benefits

The pay range for this position is $50.00 - $55.00
