*JOB DESCRIPTION *Title: Assistant Physician - Primary Care **Report To: Executive Director and/or Medical Director

**Summary: We are looking for an Assistant Physician (AP) to join our expanding primary care/behavioral health team and provide patients with high-quality treatment and diagnostic services. The ideal candidate is an intuitive healthcare professional whose diagnostic abilities are only matched by their concern for their patients. The person in the Assistant Physician role will be responsible for outreaching, connecting, meeting, assessing and diagnosing patients in our clinics and at patients residences and providing them with healthcare services that meet state and national standards for caregiving. AP will be responsible for prescribing patients with appropriate medication, health regimens and lifestyle adjustments that will contribute to their recovery from diagnosed health issues, prevent future health issues and/or promote a healthier life. The right candidate will have a passion for providing diagnostic and treatment services to patients of all ages and will enjoy working closely with other healthcare professionals as team members of our: a) Primary Care Health Home (PCHM); b) Behavioral Health Home (BHM); and c) other such interdisciplinary specialized evidence-based, best-practices teams.

Responsibilities for Assistant Physician

Work with the a) Primary Care Health Home (PCHM); b) Behavioral Health Home (BHM); and c) other such interdisciplinary specialized evidence-based, best-practices teams to provide patients with medical

and behavioral care services within the standard of care for patients.

Order laboratory and radiology tests and refer patients to specialists when appropriate and called for.

Analyze reports, test results, medical records and examinations to diagnose condition of patients and provide appropriate treatments.

Prescribe pharmaceuticals, other medications, and treatment regimens to treat identified and documented medical and behavioral health conditions.

Discuss potential side effects of medication/treatment with patients and ensure that all allergy information is up-to-date before prescribing medication/treatment.

Provide advice to patients for lifestyle and diet changes that may improve their health or help to treat the health issue they are experiencing.

Adhere to departmental policies, procedures and objectives, ongoing quality improvement objectives and safety, environmental, and infection control standards.

Must not have another job that conflicts with current position at HTC or that interfere with full and engaging responsibilities at HTC, such as working external private job or with competitor/s and be tired or spent to function effectively in HTC responsibilities.

Any private practice or work with competitors (within or outside official work hours) must be fully disclosed, discussed and agreed in writing.

Maintain patient confidentiality and comply with all federal and state health information privacy laws.

Record complete, timely and legible medical

records.

Work closely with the collaborating physician.
