Sweetser is looking for a Clinician for our Child ACT Program in Saco! This role provides the opportunity to work in a multi-disciplinary team to support youth with serious emotional disturbance and their families. The team consists of a clinical team lead, psych nurse practitioner, case manager, family support specialist, youth peer support specialist, and a behavioral health specialist. This program is slated to provide services for 25-50 youths and their families within a 25-mile radius of our Saco Campus. Services will be delivered in the home, community, or office setting depending upon what is most therapeutic for the youth and family. This position has the responsibility for ensuring the delivery of the highest quality of services to clients and supporting the coordination and integration of available services and resources on behalf of clients in on the ACT team.
ESSENTIAL FUNCTIONS:
Collaborate flexibly and responsively with the area service network on behalf of clients to support smooth access to the program, coordinated service responses and client linkage with needed community resources.
Support and articulate the program and organization mission to staff, clients, providers and others.
Offer trauma informed other best-practice clinical techniques and therapies.
Provide customer service within established deadlines, consistent with organizational values, standards, and policies.
Demonstrate leadership in case-coordination.
Complete timely, professionally written, intake assessments.
Manage insurance authorizations in a fiscally responsible manner.
Use quality Collaborative Documentation to maintain insurance required documentation.
Schedule clinical appointments to meet the clinical needs of the client.
Use organization’s established tools and procedures to formulate assessment including diagnoses of the child/adolescent and his/her family. This includes level of care and risk assessments as needed. Involve family members and significant others.
Develop an individualized service plan with the individual and natural supports.
Identify concrete service needs and provide integrated treatment with the client’s natural supports and providers to demonstrate best-practice.
Ensure that the program meets the ACT fidelity standards
EDUCATION REQUIREMENTS:
Masters degree in professional clinical discipline.
CERTIFICATES, LICENSES, REGISTRATIONS REQUIRED:
Independent licensure preferred - LCSW/LCPC will consider LCSW-cc or LCPC-cc.
Valid state driver’s license at time of hire.
