Position Overview:
Conducts clinical evaluations, assesses patient referrals, and facilitates admissions through collaboration with other rehabilitation disciplines and physicians. Serves as the primary contact for referrals from physicians, external agencies, hospitals, case management (internal and external), and caregivers, including family members. Fosters and builds referral network by educating resources on the advantages and benefits of Sutter's Acute Rehabilitation Program, executing ongoing marketing activities, and developing working relationships with hospitals, businesses, allied health professions and community-related groups. Helps maintain the inpatient census for Rehabilitation through sustainable relationships with referral sources.

EDUCATION
Bachelor's or equivalent education/experience
OR Other: Graduate of an accredited school of nursing.
CERTIFICATION & LICENSURE
RN-Registered Nurse of California
OR PT-Physical Therapist
OR OT-Occupational Therapist
OR SP-Licensed Speech Pathologist
EXPERIENCE
5 years experience in clinical practice.
SKILLS AND KNOWLEDGE
In-depth knowledge of acute rehabilitation or similar healthcare service insurance criteria and admission guidelines to determine patient eligibility.
Working knowledge of medical terminology and the relationship to the rehabilitation process.
Demonstrated familiarity with local/state/federal regulations, insurance guidelines, as well as, Sutter Health’s protocols as they apply to acute rehabilitation.
Possesses proficient written/verbal communication, interpersonal, and presentation skills to explain medical procedures or sensitive information clearly and professionally to diverse audiences.
Good analytical and project management skills, including the ability to analyze problems, reach practical conclusions, and institute effective changes.
General computer skills, including a working knowledge of Microsoft Office Suite and EPIC to create/update patient electronic health records.
Work independently as well as be part of an interdisciplinary team, while demonstrating attention to detail and solid organizational skills.
Identify and solve complex problems based on the analysis of multiple sources of information while providing new perspectives on existing solutions.
Ensure the privacy of each patient’s Protected Health Information (PHI).
Evaluate the competition and identify the factors that differentiate them in the market.
Build collaborative relationships with patients, their families/care givers, other healthcare providers (physicians, nurses, etc.) and community resources/agencies to coordinate efforts and provide the highest quality of patient care.
License/Certifications:
