Job Details Program Overview: Full oversight to support Catholic Health’s Home Infusion Pharmacy Growth. Identify, outreach, and educate current and potential referral sources. Foster relationships with current and potential referral sources. Conduct in-services to introduce services, therapies, drug detailing, and products to referral sources. Responsible for branding/marketing to all potential home infusion customers. Collaborate with CH specialty pharmacy, ambulatory infusion centers, payers, physicians, clinicians, referral sources, and home health agencies to coordinate patient care to the home. Identify and oversee patient referral process. Collaborate, coordinate, and communicate with the intake team to provide details needed for new patient referrals and ensure seamless patient transition to home infusion services. Follow up with all new referrals and maintain patient care communications with referral sources, physicians, and internal/external teams. Monitor and oversee daily Home Infusion Pharmacy intake operations. Oversee and monitor compliance and regulatory requirements, including partnered nursing agency documentation. Drive activities related to customer pull-through, process improvement, and ongoing communications for business growth. Identify new opportunities to optimize the Home Infusion Pharmacy program. Utilize data sources to create physician outreach/marketing plans. Collaborate with Specialty Pharmacy for program success. Monitor and report program status and KPIs to leadership. Willingness to travel up to 50% of the time to referral sources. Qualifications: Bachelor’s degree in Business, Healthcare, or related field. 2+ years of experience in Home Infusion, Home Health, or related field. 4+ years of Project Management experience. Knowledgeable in Specialty Pharmacy line of business. Experience working directly with Physicians, Nurses, and clinical staff members. Payer knowledge and ability to navigate Medicare, Medicaid, and Commercial insurance plans. Experience in specialty therapy marketing. 2+ years of strategic planning experience; territory optimization execution is a plus. Salary Range USD $100,000.00 - USD $120,000.00 /Yr. This range serves as a good faith estimate and actual pay will encompass a number of factors, including a candidate’s qualifications, skills, competencies and experience. The salary range or rate listed does not include any bonuses/incentive, or other forms of compensation that may be applicable to this job and it does not include the value of benefits.
