Coordinated Care Services Inc (CCSI) (https://positions.paylocity.com/PositionManagement/wizard/2033511) seeks a talentedService Access Coordination Managerto contribute their expertise in providing essential business services to our provider organizations and customers. TheService Access Coordination Managerprovides oversight and monitoring of the Rapid Engagement Delivery (RED) Program, Safe Options Support (SOS) Critical Time Intervention (CTI) HUB Coordinator, and SAMHSA Homeless Partnership Project. Service Access Coordination Manager collaborates with community partners to identify and address needs and barriers in the homeless/at risk of homelessness community. Position fosters and develops collaborative relationships with Monroe County Agencies to address homelessness, social determinants of health, linkage to stabilization services, and mental health or substance use disorder services.

In this role, you will

  • Manage day to day operations and technical support of RED Program, SOS CTI Grant, and SAMHSA Projects, including but not limited to: conducting meetings, providing leadership to quality improvement process, promoting open communication, and resolving ongoing problems and concerns of consumers, community partners and staff.

  • Provide case management and conduct home visits as appropriate.

  • Participate in case reviews and coordination with continuum of care.

  • Develop project operational policy, procedures, protocols, workflows, MOU’s, contracts, and partner relationships; secure/schedule training and ensure adherence to Task Plan.

  • Manage pool of funding to purchase short term stays in variety of residential settings for SAMHSA project.

  • Evaluate program effectiveness to develop improved methods; devise evaluation methodology and implements; analyze results and recommend and/or take appropriate action. Work towards effective sustainability of programs and funding.

  • Conduct presentations on program/project to community agencies and stakeholders.

  • Supervise and professionally develop staff members.

  • Respond to informational inquiries regarding referral process and basic aspects of project services.

  • Participate in continuous improvement process for county processes, as requested by supervisor.

  • Facilitate collaborative processes and maintain communication among service providers and with the community.

  • Incorporate recognition of diverse clientele in daily activities

  • Respect the confidential nature of certain information exposed to in the course of work.

  • Review and revise processes as needed. Update policies and procedures.

  • Act as representative of CCSI, and Office of Mental Health in key stakeholder meetings.

  • Travel as necessary to fulfill job responsibilities.

  • Responsible for timely submission of all data and reporting as required for each grant/funder.

  • Oversight of grant budgeting and finances, as well as grant and contract renewals and future consideration of sustainability efforts.

  • Other duties as assigned.

Requirements

  • Bachelors in mental health, social work, or related human services and 5 years’ experience in CD/MH Field, or equivalent education and work experience (see CCSI's equivalency chart (https://www.ccsi.org/CCSI/media/pdfs/Equivalency-chart.pdf) )

  • Experience with Windows based operating system (Word processing, spreadsheet and database management software)
