The referenced pay range represents the minimum and maximum compensation for this job. Individual annual salaries/hourly rates will be set within the job's compensation range, and will be determined by considering factors including, but not limited to, market data, education, experience, qualifications, expertise of the individual, and internal equity considerations.
Responsibilities:

JOB DUTIES AND RESPONSIBILITIES: - Practices within the scope of practice intended by the Licensed Registered Nurse regulations of the New York State Nurse Practice Act. Provides direct patient care to families according to community standards for outpatient care, Primary Care Network protocols, procedures and guidelines, and professional nursing standards of care. - elephone triage utilizing professional nursing assessment skills and identified triage standards in decision- making regarding the urgency of patient needs. - Telephone follow-up with patients utilizing professional nursing assessment skills and/or as directed by providers. - Administers treatments and medications, patient teaching and health education directly to patients in collaboration with provider visits. - Documents all patient interactions. - Supervises LPNs and medical assistants as assigned. - Participates in formal and informal residency education during patient care sessions, team meetings. - Performs other tasks as requested or assigned to the function of registered nurse. QUALIFICATIONS: Licensed and registered by the New York State Education Department (NYSED) as a registered professional nurse (RN) to practice nursing as an RN required. Must comply with NYS RN BSN requirements. One to two years of experience in direct nursing care, experience with patients of all ages, Ambulatory care and telephone triage experience preferred or equivalent combination of education and experience. Computer literacy and effective interpersonal communication, problem solving skills, ability to work collaboratively and courteously with others required. Strong leadership skills required. Certification as a Licensed Registered Nurse in New York State, NYS Licensed RN, CPR, BLS for Healthcare Providers (AHA) required.
