RPH is seeking an RN to take on a leadership role as the RN Memory Care Case Manager at our Memory Care Residence located on Thurston, Road in Rochester. Job Description
The primary purpose of this position is to plan, organize, develop, and direct the overall process of resident case management for elders in a Memory Care Assisted Living setting, in accordance with current federal, state, and local standards and regulations to ensure that the physical, emotional, and social needs of the residents are maintained on an individual basis.
The RN Case Manager serves as a member of the interdisciplinary team and is specifically trained in the supportive care of persons with dementia.
Job Responsibilities:
Ensure that the Assisted Living ALR and Enhanced Care EALR provides an environment that encourages independence and aging in place as appropriate.
Maintain documentation in accordance with the Department of Health Guidelines for all levels of Assisted Living.
Plan, develop, organize, implement, evaluate, and direct case management activities caseload.
Responsible for the plan of care and the outcome of that care, including care delivered by other nursing team members who must be directed and supervised by the RN Case Manager.
Evaluate and screen resident re-admissions as requested.
Provide Dementia specific collaboration in programing to meet the ever-changing needs of the resident.
Ensure that resident progress notes are informative and descriptive of services provided and of the resident’s response to the services.
Focus on the total well-being of the resident, which includes social, family, financial, medical, mental health, key life events, dietary, and spiritual.
Ensure quality of care through regular daily reviews of required documentation, physician’s orders, chart audits, and overall individual service plan completion and review.
Observe signs and symptoms and report to the physician reactions to treatments, including drugs and changes in the patient’s physical or emotional condition.
Collaborate with the Administrator and the nursing care team to maintain Department of Health regulated records, census, incident report, admission, and discharge information.
Support and update family as appropriate on resident’s status.
Stay current with healthcare policies, practices, and evidence-based guidelines to provide the highest quality of care to residents.
Teach, supervise, and counsel the residents and caregivers regarding nursing care needs and other related problems.
May also assist with providing direct care and medication pass as required when staffing levels are challenged.
Attend and participate in Resident & Family Council Meetings as needed.
Ensure that the Assisted Living ALR and Enhanced Care EALR provides an environment that encourages independence and aging in place as appropriate.
May participate in recruitment, selection, and training of staff with input and assistance from other leaders.
Responsible for completion and coordination of all resident assessments with responsible parties, to ensure adherence with all timelines and procedures and to ensure appropriate placement in the ALR according to admission and retention criteria, with support from Family Care Team.
Assist with facilitating a safe and successful transfer or admission of a resident to their environment, in coordination with other personnel.
Responsible for ensuring the environment is well-maintained, safe, and homelike in accordance with appropriate department managers.
Requirements:
NYS Licensed RN
At least three years’ experience working with an adult populating providing supportive care services in an adult care or long-term care setting.
Experience with Dementia specific care.
Proven experience in case management, care coordination, or care management.
Strong critical thinking skills, the ability to follow standards/procedures, and make appropriate clinical judgements to provide safe and effective care.
Ability to work independently with minimal supervision, adapt to change, and manage a variety of assignments while establishing priorities and ensuring the necessary duties are completed.
Excellent communications skills, both written and verbal.
Ability to maintain confidentiality and handle sensitive information with discretion.
Physical Requirements:
The physical demands described here are representative of those that would need to be met by an employee to successfully perform the essential functions of this job in an adult memory care assisted living environment. While performing the duties of this job, the employee is occasionally required to stand; walk; sit; use hands to finger, handle, or feel; and talk or hear. Prolonged periods sitting at a desk and working on a computer. The employee may occasionally lift and/or move up to 25 pounds, exert up to 50 pounds of force occasionally, and/or a negligible amount of force frequently or constantly to lift, carry, push, pull or otherwise move objects, including the human body. Specific vision abilities required by this job may include close vision, distance vision, color vision, peripheral vision, depth perception, and ability to adjust focus.
