Job Summary: The Delphi Rise Monroe County Reentry Program is a small but mighty team, and we are looking for our next rock star team member! We proudly serve individuals who are returning to our community from state prison and are in need of tailored services and supports for a successful transition. Our Reentry Program works closely with key stakeholders to ensure that our services impact the community on the individual level AND the systemic level. We are looking for someone who is passionate about justice, excellent at collaborating, driven to overcoming systemic barriers, and willing to lead a group of dedicated individuals. This role will be a hybrid position requiring supervision of staff as well as the oversight of a small caseload, allowing the successful candidate to stay close to the issues and work.

Essential Job Duties and Responsibilities: (Additional duties may be assigned) Review assessments, Service Coordination Plans, and case notes to ensure deadlines are met and program policy and procedures are understood and followed. With the support of the Program Director, provide training to staff and orient new employees to agency policies and program requirements. Assist clients in obtaining benefits, employment, treatment, housing, childcare, identification paperwork, and other key supports. Coordinate client care with health care providers (primary care doctors, treatment counselors, etc.). Coordinate client services with NYS Dept. of Corrections and Community Supervision (Parole), Monroe County Probation, the Court system and other collaborative partners and stakeholders. Create a comprehensive service plan for each client, providing necessary resources and support for successful completion. Maintain accurate and timely client records and case notes. Actively participate in Monroe County Reentry Task Force meetings. Other job duties as assigned by management.

Supervisory Responsibilities: The Reentry Program Supervisor will provide direct supervision to Care Managers and the Employment Specialist, including regular one to one supervision meetings, performance evaluations, and professional development opportunities.

Minimum Qualification Standards: Two years prior supervisory experience in a human services setting At least five years of experience working with individuals who have a history of serious persistent mental health, substance use disorder and/or criminal justice history. Experience with the criminal justice system is highly valued.
