About Us:

East House is a leading healthcare and support services provider, dedicated to helping individuals transition to more independent community living. We are currently seeking a dedicated and qualified Health Care Coordinator to join our team.

Position Overview:

As a Health Care Coordinator at East House, you will play a vital role in providing medication, physical health, and nutrition education and support to our residential clients as they work toward a more independent lifestyle. You will report to the Program Manager and collaborate with medical providers, ensuring our clients' health care needs are met effectively.

Key Responsibilities:

  • Partner with assigned clients to assist in getting medical needs met.

  • Collaborate with external medical providers to facilitate client needs being met.

  • Provide education and support to clients and their supports about medical issues and needed follow-up.

  • Act as a liaison for the unit, working with pharmacies and doctors when questions arise.

  • Participate in the development and maintenance of the therapeutic community.

  • Maintain familiarity with all clients in the unit, actively participating in Case Reviews and reviewing communication logs.

  • Address the needs of the residents in the unit that occur during on-duty shifts.

  • Track when all client physicals and PPDs are due and support counselors in ensuring appointments are made prior to lapse.

  • Provide 1:1 attention and transportation for medical appointments when clients need added support.

  • Provide monthly training to all residents on topics such as Fire safety, Personal Safety, Nutrition, and Tobacco Cessation.

**This is a Full-time Position, working 35 hours per week.

Requirements

Qualifications:

  • Minimum Education/Experience:High school diploma plus 5 years of experience or equivalent educational and experiential qualifications.

  • Licensure/Certifications:NYS motor vehicle license, a safe driving record, and availability of a personal vehicle for work.

  • Other Skills/Knowledge/Experience:

  • Effective written and oral communication skills.

  • Demonstrated strong organizational skills.

  • Ability to work cooperatively and effectively with culturally diverse clients, staff, and community service providers.

  • Basic computer literacy, including proficiency in Microsoft Word for basic tasks.

  • Physical Demands:

  • Job requires sitting, handling objects and paperwork, working with a computer.

  • Occasional lifting (up to 30 lbs), walking, standing, and reaching.

  • Finger dexterity for operating a telephone and computer keyboard.

  • Hearing to enable normal conversation.

  • Visual requirements to read documents and a computer screen.

  • Ability to maintain emotional control under stressful situations.

  • Working Conditions:Respond to crises and emergencies during on-duty hours, occasional travel in Monroe County, and occasional transportation of clients.

Salary Description
