Salary Range: $65,000 - $75,000 (Pay information will vary depending upon relevant experience for the position.)

The Director is responsible for the supervision of the social work and therapeutic recreation departments at the skilled nursing facility to meet the psychosocial, recreational and social needs of the individual residents which will provide needed services, assure quality of life, and ensure compliance with regulations and policies.

ESSENTIAL JOB FUNCTIONS

  • Direct the social work and recreation programming to meet the psychosocial, emotional and individual needs of the residents in accordance with State and Federal regulations.

  • Ensure that the facility has written policies and procedures regarding residents’ rights in compliance with all State and Federal regulations.

  • Establish departmental practice standards and assure quality service by adhering to current clinical standards of care.

  • Coordinate interdisciplinary case management protocols to be utilized between various levels of care and departments.

  • Work with committees and quality teams in furthering the mission of ECH.

  • Assist admissions in coordinating residents’ room assignment to meet the needs of the resident and that of the facility.

  • Represent the organization in its relationships with other case managers, social workers, service organization, professional associations and other health care providers as it relates to case management and recreation.

  • Coordinate resident council and family meeting schedules.

  • Provide in-service training for staff on resident rights and abuse prevention and update policies on above as needed.

  • Serve as a department head responsible for the development of organizational planning, including departmental budgeting, and the development of departmental policies and procedures.

  • Ensure the completion of required documentation for assigned residents including progress notes, assessments, MDS sections and care plans, as needed.

  • Recruit, select and supervise the staff of Social Workers and Therapeutic Recreation staff by providing on-going consultation and clinical support.

  • Oversee special events programming including responsibility for holiday programs and decorations.

  • Establish Social Work and Recreation caseloads for the employees in the department.

  • Participate in appropriate community workshops and conferences to enhance professional growth.

  • Provide in-service education and informative presentations to staff, families, volunteers and community organizations as requested.

Requirements

QUALIFICATIONS:

  • Ability to interact courteously and tactfully with staff, family, visitors, vendors and the general public. Demonstrate success as a leader. Demonstrate knowledge in the field of gerontology. Demonstrate excellent communication skills, customer focus and creative problem solving.

  • Masters’ degree in social work from an accredited graduate school and a current certification from NYS Education Department to practice social work.

  • Three to five years of experience in health care or senior care.

SUPERVISORY RELATIONSHIP:The work is performed within established guidelines, and requires periodic supervision. The employee exercises initiative and independent judgment to effectively perform the essential job duties of this position.

RESPONSIBILITY FOR OTHERS:The employee has direct responsibility for others.

SAFETY RESPONSIBILITIES:Shall be fully acquainted with all safety policies and procedures of the Church Home. Takes an active role in all fire drills during the shift and/or emergencies for internal and external disasters as monitored by supervisor and incident manager. Must comply with facility in-service requirements.

Maintain confidentiality of all information related to the organization, residents, participants, family and staff, that may be encountered, either formally or informally, during the normal course of business.This includes medical and treatment records, financial and human resources information.
