Position Overview: Provides occupational therapy services to residents in the program, under the supervision of an Occupational Therapist, Registered (OTR); provides consultation and any required clinical supervision related to the provision of OT services or other Individual Program Plan (IPP) components for which responsible, functioning as an Interdisciplinary Team (IT) member and assuring compliance with all applicable residential standards.

Responsibilities:

  • Provides initial and annual occupational therapy assessments if the individual's medical assessment indicates a need and is required by the physician or if requested by the Interdisciplinary Team (IT). These assessments shall follow the format established by the Program and have the approving signature of the supervising Occupational Therapist, Registered (OTR).

  • Develops goals, objectives and methods to address occupational therapy problems or needs and to maximize assets or strengths, for inclusion in the individual's IPP, with approval of the OTR and in collaboration with relevant IT members.

  • Provides occupational therapy services on a regular basis to achieve IPP goals and objectives which are designed to: Preserve and improve gross and fine motor development, range of motion, strength, muscle tone, endurance, coordination, perceptual and sensorimotor skills, self-care, and daily living skills, mobility and B. Prevent, to the extent possible, progressive disabilities through the use of purposeful activities, orthotics and prosthetic devices, assistive and adaptive equipment, positioning, and sensory stimulation. C. Provide and assure integration and coordination of occupational therapy services with the individual's place of Day Program when in his/her best interest and to the fullest extent possible.

  • Participates in developing and implementing IPP components which require IT input and responsibility.

  • Participates in all individual Initial, Annual and Quarterly Reviews as determined by the IT and monthly Team Meetings.

  • Fulfills all record keeping and documentation requirements including at least Progress Notes, Clinical Record Notes, Monthly Summaries and Review Reports.

  • Determines needs in collaboration with the OTR and submits requests to the QMRP Coordinator for equipment and supplies necessary to provide occupational therapy services.

  • Participates in staff meetings: provides input to program evaluation and development and participates in education and training opportunities, as requested.

  • Fulfills all Program Individual Coordinator responsibilities for those individuals assigned under the supervision of a QMRP.

  • Adheres to all agency and program policies and procedures.

  • Performs all other necessary duties relevant to the position as requested by Program Director, QMRP Coordinator Occupational Therapist and approved by the Executive Director.

  • Advises the QMRP/Occupational Therapist of any obstacles to achieving responsibilities.

    Qualifications:

  • Licenses and currently registered as a Certified Occupational Therapy Assistant by the New York State Department of Education. One (1) year of experience working with the developmentally disabled is preferred.
