Case Manager Housing Supportive Services

Job Summary:
You will provide case management services, including, but not limited to, screening, intake, orientation, assessment, crisis intervention, client education, referral, report and recordkeeping.
What You Will Do:
Act as a "first responder" addressing complex crisis involving both psychological and environmental components
Work holistically, ensuring that care and discharge plans meet the physical, social and emotional needs of the client
Coordinate all of the client resources, as needed
Assist individuals with housing assistance
Complete a comprehensive assessment of the client's needs and identifies resources, including basic needs and assists in accessing these services
Document all services in the clinical record, with all entries authenticated and time/dated
Create a care plan with client
Link clients to community resources, as needed
Maintain ethical boundaries with clients and staff
Maintain client confidentiality as per state regulations and agency policies
Provide support for team colleagues, and engage in program-related performance improvement processes and activities
Engage in inter-organizational care coordination and follows up on referrals when needed and/or indicated
Attend regularly scheduled multidisciplinary team meetings (MDT), as assigned
Prepare and submit documentation required for billing authorization as needed
What We Need to See:
Bachelor's degree in health, psychology, counseling, social work, education or other behavioral health profession from and accredited college or university
Must have experience and skill in competently performing case management activities
Must have at least one (1) to two (2) years' experience working with the homeless or at-risk population and/or with high need individuals with SMI/COD/SUD
Valid driver's license and must be insurable by agency's automobile insurance for primary drivers.
