East Bay Community Action Program Transition of Care Registered Nurse in Riverside, Rhode Island

East Bay Community Action Program (EBCAP) is seeking a full time Transition of Care Registered Nurse for the 100 Bullocks Point location in East Providence, RI. We are seeking a highly professional and motivated individual who enjoys working with clients in a community health setting. Join our expanding dynamic multidisciplinary team and provide comprehensive patient care using a care team model.

The successful candidate will review and abstract all incoming transitions of care documents from patient emergency department visits and hospitalizations with 24 to 48 hours of receipt. These include, but are not limited to medication reconciliation, reason for visit, allergies, immunizations, diagnostic imaging and pertinent lab results. Educate patient about when to call provider first before going to emergency room to decrease unnecessary utilization. He/she will schedule follow up appointments for all inpatient visits with patients primary care provider within 7 - 14 days of discharge depending on complexity. He/she will review high risk patient lists and communicate with care management team and care team for nurse care manager.

The successful candidate will have a minimum of an Associates Degree in Nursing or equivalent and be licensed as a Registered Nurse by the Rhode Island Department of Health. Two of more years experience in a community health setting, acute care or emergency/urgent care working with primary care providers to coordinate care and disease management required. Previous experience in an electronic medical record (EMR) preferred; however, would receive training in EBCAPs EMR which is NextGen.
