Actalent RN Case Manager in Riverhead, New York

Job Title: Case Manager

Description

We provide the highest quality of comprehensive primary, preventative, and behavioral health services to all who seek it, regardless of insurance status and ability to pay, especially for the underserved and vulnerable. We are a Federally Qualified, Non-Profit Health Center serving communities in New York.

Responsibilities

  • Conduct intake assessments with patients and families to understand their needs and develop individualized treatment plans.

  • Ensure that families feel sufficiently equipped and supported throughout the treatment process.

  • Develop and adjust treatment protocols as needed, considering physical, financial, and other constraints.

  • Coordinate care plans and work with other healthcare providers to ensure that patients receive the necessary care.

  • Monitor patients’ recovery progress and update care plans as necessary.

  • Maintain accurate and up-to-date patient records and documentation.

  • Provide education and support to patients and their families regarding health conditions and treatment plans.

  • Collaborate with medical staff and other team members to provide high-quality care.

Essential Skills

  • Registered Nurse License - NYS

  • CPR Certification from either the AHA, ARC, or National CPR

  • Bachelor's degree in Nursing

  • Infection control

  • Case management

  • Medical care management

Additional Skills & Qualifications

  • Ability to communicate in English, both orally and written

  • Possess supervisory experience

  • Knowledge of nursing standards and competence in nursing skills

  • Experience with substance abuse population

  • Strong commitment to public and community health

  • Ability to work with individuals from a variety of ethnic and socioeconomic backgrounds

Work Environment

Training takes about 90 days and will be fully onsite. Once training is complete, you can work 2 days remotely. Our work environment is community healthcare oriented with great benefits when transitioned to a permanent employee. We offer opportunities for growth and education, promote from within for long-term success, and provide tuition reimbursement up to $2,000 per year for non-union locations after one year of employment. This position has full benefits and direct deposit weekly pay.
