Responsibilities:

The Housing Coordinator will be responsible for: 1) developing and maintaining external relationships with housing and other Social Determinant of Health (SDOH) focused agencies and organizations; 2) leading efforts to identify housing and SDOH opportunities for members; 3) educating, training and consulting internal teams on SDOH resources and systems that are appropriate to members’ needs; and 4) assisting with complex cases for members who are at risk of or experiencing homelessness or require additional SDOH supports to access, improve and stabilize their situation.

The Housing Coordinator will have expertise in navigating federal, state and local housing programs and in understanding the landscape of residential choices for members. The Housing Coordinator will have previous experience in housing case management and services, and advise Care Managers and other member-facing teams on how to assist members in finding and retaining and affordable, accessible housing in the community.

Other key responsibilities for the Housing Coordinator include:

  • Establishing –and evaluating - innovative partnerships with housing and SDOH organizations to develop and increase access to affordable and accessible housing options, particularly for those who wish to transition to the community.

  • Partner with community based organizations to develop and implement training specific to affordable housing services. Creating a culture of housing at through on-going trainings and consults to Population Health in identifying housing options for members.

  • Serving as subject matter expert and organization resource in housing and representing the social determinant of health and healthy opportunity needs for members.

  • Developing standards and procedures for staff used to close the loop on resource connections to housing and other in-home supports.

  • Representing the business in community meetings and workgroups relevant to housing issues of our members and will help secure housing for those members experiencing housing insecurity and homelessness.

Education/Experience:

  • Bachelor’s Degree preferred

  • Associate Degree or equivalent work experience required

  • The Coordinator must be familiar with the relevant public and private housing and SDOH resources and stakeholders.

  • Ability to leverage knowledge and experience of affordable housing programs for identified populations at a local, State and federal level to identify and develop housing opportunities.

  • A minimum of 3 years of experience in direct services (housing) for vulnerable populations, non-profit or government program management (housing services), or both.

  • Ability to employ Motivational Interviewing and other person-centered approaches to engaging members in housing navigation services

  • Strong written and verbal communication skills.

  • Must also be able to provide effective training, prepare written reports, and maintain client documentation.

  • Ability to effectively present information and respond to questions from staff, members, funders, governmental agencies, and the general public.

  • Demonstrated knowledge of the system of housing opportunities and how to access them (e.g., Coordinated Entry system, public housing, eviction prevention/rental assistance programs, legal assistance, affordable housing applications / re-certification process, etc.).

  • Ability to work in various communities and travel throughout the State of North Carolina required.

  • Valid Driver’s license.
