JOB SUMMARY:

Background: The Office of Mental Health (OMH) and the Department of Health (DOH) are co-defendants in the Stipulation and Order of Settlement, United States v State of New Your, Civil Action No. 13-0CV-4165 (NGG) O’Toole et al v. Cuomo et al., Civil Action No. 13-CV-4166 (NGG). This settlement requires the state to assist residents of impacted adult homes, who have been identified as “class members” to move out of adult homes into supported apartments if they choose to and are eligible. They will have the assistance of a care manager to ensure any needed wrap around services are in place prior to the move. An assessment determines the care planning needs that the care manager implements as part of the preparation for transition out of the Adult Home.

The Registered Nurse Assessor, in collaboration with the Social Work Assessor and assigned Care Coordinators, will be responsible for working with other team members to streamline the assessment and placement into supported housing, of identified residents of impacted Adult Homes. The Registered Nurse Assessor will have primary responsibility for assessing the physical health needs of the residents— for example Managed Long-term Care services, Certified Home Health Agency services, etc.

ESSENTIAL JOB FUNCTIONS:

  • Coordinates and collaborates with the Social Worker Assessor, multi-disciplinary team members, Adult Home staff, and Care Coordinators toward the goal of transitioning residents into independent living arrangements

  • Provides comprehensive nursing assessment of the health status of residents, and develops nursing diagnoses, and care plans

  • Implements nursing strategies or interventions according to the established care plan;

  • Evaluates resident’s and caregiver’s responses to care plan for achievement of realistic client-centered outcomes and advises Care Manager as appropriate

  • Documents resident’s responses to care plans

  • Describes and documents nursing processes clearly, concisely, accurately and appropriately, according to ICL and OMH Care standards

  • As assigned, prepares reports in a timely and accurate manner

  • Performs other job related duties that may be assigned.

ESSENTIAL KNOWLEDGE, SKILLS AND ABILITIES:

  • Strong assessment and analytical skills, and a commitment to person-directed care

  • Excellent interpersonal, organizational, time management, decision making, and problem-solving skills;

  • Excellent written and verbal communication skills, including proficiency in computer applications, especially Microsoft Office and electronic health record programs

  • Ability to maintain confidentiality;

  • Ability to follow direction and work within the policies, procedures , mission of the agency and of the Adult Home Supported Housing Project.

QUALIFICATIONS AND EXPERIENCE:

NYS license and valid, current registration as a Registered Professional Nurse. At least 5 years’ experience providing direct services to individuals with mental illness. Must be highly skilled at performing physical health assessments and developing and monitoring appropriate plans of care.
