    • Qualified NYS licensed Podiatrist to perform part-time (2 visits per month, 5 hours a visit, totaling approximately 10 hours a month – seeing on average 20 – 25 patients per visit) Podiatry Services on site for Creedmoor Psychiatric Center (CPC) clients. Hours are subject to change.
Scheduling of clients to be treated will be arranged by the Facility Medical Clinic. No requests for services will be honored unless authorized by the treating physician. The Facility will provide contractor with pertinent medical information regarding the patients that are being referred to the contractor including, but not limited to, a brief history. The Contractor, in coordination with the Facility Medical Clinic, will be responsible for the following:  Provide general podiatry services to CPC patients, including evaluation and treatment recommendations for podiatry conditions and foot pain.  Debridement/trimming and partially or completely removing toenails.  Shaving/trimming hyperkeratosis (corns and calluses)  Referring patients to outside providers for surgical interventions, as required  Incise/drain abscess, debride ulcers, treat warts.  Inject local anesthetics, steroids, dilute alcohol, etc.  Prescribing interventions, including medications and treatment.  Providing additional podiatry services, as requested.  The vendor agrees to mutually acceptable daily sign-in/out documentation procedure. A log of patients C# & the service & the time maintained. This will be signed off by both the vendor & the facility coordinator.  There will be no reimbursement for travel expense or travel time. The Facility will provide exam rooms, treatment chairs, instruments and medical procedure supplies as required to provide podiatry services. In addition, the Facility will provide a nurse to assist the Contractor. The assistant will be responsible for directing the patients, preparing the equipment and supplies, cleaning up the treatment area after each session, assisting the physician with the completion of the proper forms, cleaning of instruments, exam table and chair, and providing general support/safety to patients and the Contractor. All OSHA requirements, which include the proper sterilization of all podiatry instruments for the protection of each patient, shall be observed and practiced. All instruments for use by the Contractor shall be furnished by the Facility. All medical documentation shall become the property of the Facility.
