Responsible for supporting communities as part of CommonSpirit Mountain Region Health’s Community Health Advocate (CHA) team. CHA’s are public health workers and trusted members of the community. They serve as a link between patients and social service agencies in the community to help members gain access to available health programs. Provide consumers with information and assistance to access the health care and enroll our community members in public insurance programs. Utilize knowledge and skills in community health improvement and care management to support the Community Health Needs Assessment Implementation plan

Qualifications:

In addition to bringing humankindness to the workplace each day, qualified candidates will need the following:

  • Two years of experience working in a Community Health Worker (CHW) role or experience with social services, support services, public health or related health outreach, preferred.

  • Experience interviewing and collecting confidential data and maintaining records.

  • Experience in completing standard forms; previous experience with Connect for Health Colorado enrollment, a plus.

  • Written and oral fluency in English

  • HS GED (Required)

  • Bachelors Preferred

  • Previous Community Health Worker training preferred

  • Bilingual in Spanish (preferred)

Sedentary work -(prolonged periods of sitting and exert up to 10lbs force occasionally)

This position is hybrid role. Applicants residing in Utah’s Wasatch Front will be given priority (must reside in UT, CO, KS, or NM). This is a grant funded* position focused on providing outreach and assistance to patients at risk of food insecurity. Prior experience with health and human services and working independently and/or in a remote or hybrid role are strongly preferred. *CommonSpirit Mission and Ministry Fund grant ends 6/30/2026. We are interested in applicants to have experience and knowledge of Medicaid, SNAP food assistance and all other Human Service programs that are available in Utah.
