POSITION SUMMARY: The Patient Care Navigator (PCN) works as an integral part of the care management team acting as an educator, resource and advocate for patients and their families. The PCN is responsible for navigating and coordinating care for urological cancer patients. DUTIES AND RESPONSIBILITIES

  • Collaborate with patients and care teams to develop patient-centered care plans, with a focus on disease management, prevention, and overall wellness.

  • Identify and engage appropriate patients for care management from lists and referrals, in collaboration with physicians and supervisors.

  • Assist patients and/or caregivers in scheduling patient appointments and accessing services as needed.

  • Collaborate with community partners to facilitate patient and caregiver access to community resources relevant to the patient’s needs.

  • Identify resources available for patients that may enhance their self-care management.

  • Assist providers, staff, patients, and family on the completion of enrollment forms for cancer therapies (free drug, infusions, and foundation funds).

  • Performs administrative tasks including but not limited to; completion of forms, prescription refills according to physician protocols, sending referrals, communication with patients, obtaining prior authorizations, and chart review.

  • Responsible for organization, cleaning, and re-stocking of exam rooms.

  • Demonstrates appropriate use and disposal of hazardous waste.

  • Fields patient phone calls, messages and triages, prioritizes and responds accordingly.

  • Demonstrates proper infection control procedures and follows universal precautions.

  • Serve as patient navigator for all urological cancer patients.

  • Utilize the Preveta platform to navigate and capture non face to face patient care minutes for Chronic Care Management (CCM) and Principal Care Management (PCM).

  • Coordinate and administer Bacillus Calmette-Guerin (BCG) therapy.

  • Conduct research visits as per study protocol.

  • Review and discuss study protocol procedures with the patient and family.

  • Participate in research protocol training as needed.

  • Premier Standard Operating Procedures (SOPs) are to be followed when conducting research visits.

  • Ensure adherence to Good Clinical Practices and the study protocols.

  • Ensure all laboratory results are given to appropriate doctors for review of clinical significance, then file results in the patient study binder

  • Other duties as assigned.

    EDUCATION & EXPERIENCE

  • Current NYS LPN license required.

  • Current Infection Control Certificate

  • Basic Life Support (BLS) certification is required.

    QUALIFICATIONS & REQUIREMENTS

  • Excellent organizational skills required, with the ability to communicate and work collaboratively with all levels of staff, providers, and office staff, and effectively utilizing all resources available.

  • Highly developed interpersonal skills including active listening and motivational interviewing experience.

  • Ability to assess, plan, implement and evaluate nursing care according to individual needs.

  • Able to effectively present information and respond to questions from physicians, staff and patients.

  • Completion of Good Clinical Practices (GCP) and International Air Transport Association (IATA) training as needed.

  • Able to function effectively in a team setting.

  • Travel to any of the other Premier Medical Group locations may be necessary upon request.

  • Knowledge of EMR system.
