As a Family Coach (Human Service Case Manager) you will: \' Conduct interviews to determine family needs and eligibility, and complete program applications for program benefits and, if eligible, issues appropriate support. \' Engage clients in family evaluation of strengths and jointly develop a case plan and identify the steps the family will take towards self-sufficiency and meeting their basic needs. \' Monitor progress, needs, and makes necessary modifications to the case plan. \' Initiate and participate in case staffing, employment readiness, life skills training, and supports job search as needed. \' Assist with crisis and safety planning for individuals experiencing domestic violence. \' Explore alternate resources and link clients to community services and may also assist with supporting resources and services within the community. \' Your knowledge of child welfare best practices and experience working with communities will help ensure that children and families are recognized and provided with culturally responsive and appropriate services. \' You will serve on cases and will participate on a regular basis in local activities and promote and support the well-being of families.
