UPMC Behavioral Health Counselor - CAS, UPMC Your Care in Pittsburgh, Pennsylvania

UPMC Your Care is hiring full-time Monday - Friday Behavioral Health Counselor

The purpose of this job is to provide behavioral health care coordination and disease management, with limited clinical treatment services to eligible UPMC Your members in a community, setting, with the ability to interact with members across the continuum of care. The Behavioral Health Counselor will provide services to patients with diagnoses who meet criteria for behavioral health treatment. The Behavioral Health Counselor collaborates and facilitates care with member's primary care provider, UPMC Your Care Interdisciplinary Team, other departments, providers, community resources and caregivers to achieve desired member outcomes throughout the continuum of care. Members behavioral health needs are coordinated by face-to-face, telemedicine, or telephonic interactions in their community including the hospital, providers' offices, skilled nursing facility, home, or other health care facilities.

Responsibilities:

As a Behavioral Health Counselor, you will play a vital role in coordinating behavioral health care and disease management for eligible UPMC Your members. Your responsibilities include:

  • Individual and Family/Caregiver Therapy: Provide therapy services in a community setting, supporting patients and their families.

  • Behavior Support Techniques: Demonstrate expertise in behavior support techniques to develop individualized behavior plans.

  • Functional Behavior Assessment (FBA): Conduct FBAs and design Positive Behavior Support Plans under the direction of the UPMC Your Care clinical supervisor.

  • Behavior Plan Implementation: Ensure fidelity in implementing behavior plans by staff.

  • Ongoing Evaluation: Continuously assess behavior plans for effectiveness and update as needed.

  • Clinical Supervision: Provide direct clinical supervision to assigned staff (individual, group, and onsite).

  • Interdisciplinary Collaboration: Collaborate with the UPMC Your Care Interdisciplinary Team, primary care providers, and other departments.

  • Structured Clinical Assessment: Utilize clinical assessment tools to evaluate psychiatric symptomatology, family functioning, and academic achievement.

  • Documentation: Responsible for accurate paperwork and participation in interagency team meetings.

  • Expertise Development: Develop expertise in adult/geriatric psychopathology, palliative care, crisis intervention, and behavioral therapy.

  • Care Coordination: Actively coordinate with PCP/Specialty teams and clinical resources to provide long-term behavioral health therapy/supports.

  • Referrals and Issue Resolution: Facilitate referrals for other clinical services and address service-related issues.

  • Masters degree in Psychology; Child Development; Applied Behavior Analysis; Early Intervention; Education; Special Education; Instruction in Learning; Social Work or a related field.

  • One year of clinical experience preferred. Knowledge of behavior principles, evaluation and assessment process, as well as application of current version of the DSM preferred. Licensure, Certifications, and Clearances: Behavior Specialist Licensure and/or Certification as a Board Certified Behavior Analyst, National Certified Counselor, or other similar Behavioral Health license/certification preferred. Knowledge and experience with care of the older adult, palliative care, and end of life preferred.
