Duties/ Responsibilities -

  • Working under the direct supervision of the Clinical Services Director, the primary responsibility of the RN Care Manager is to provide direct and indirect nursing care to assigned patients by utilizing critical thinking skills to assist in the management of chronic diseases, clinical problem solving and decision-making as a member of interdisciplinary team.
  • The RN Care Manager works independently in the application of advanced nursing knowledge and skills relative to NCBON scope of practice, competency and experience.
  • Participates in shared rotation of some weekend home visits (rotation every 4-6 weeks).
  • Identify patients on assigned panel who are complex and high risk in need of intensive care and/or services which may include but is not limited to education, transition of care, home visits, frequent phone calls or chronic disease interventions to prevent hospitalization and maintain continued independence in the community. Works with provider to ensure appropriate participant chronic disease management. Be proactive in identifying and coordinating potential needs of patients and/or their caregiver.
  • Completes and documents weekly visits to facility to check in on participant, monitor medication adherence and clinical care being provided.
  • Provides backup nursing care in the clinic and/or adult day center as needed per nursing protocols. Provides home visits and end of life/palliative (aka Pathways) care per nursing protocols. Participates within on-call rotation. May be assigned to float between PACE sites to assist in coverage during a critical need.
  • Works independently to assess and evaluate patient or caregiver understanding of disease process, treatment plan and/or lifestyle changes to assist in the management of health care conditions, such as hi h blood pressure, diabetes, end of life, COPD and/or CHF. Serves as a resource to the entire team for the management of these patients and conditions.

Qualifications -

  • Must possess strong interpersonal, organizational, group facilitation and consensus-building skills.
  • Must possess skills necessary to provide care for geriatric patients.
  • Must have proven clinical leadership and management skills, to include the ability to learn, teach, research, develop and implement policies and procedures and/or projects as needed independently.
  • Must possess the ability to prioritize, multi-task duties within the role as well as problem solve and think analytically/critically.
  • Must have the ability to effectively handle difficult and unusual interpersonal situations.
  • Must possess broad knowledge base of nursing clinical practice.
  • Ability to work within a team environment and foster team cohesiveness.
  • Strong oral and written communication skills.
  • Must meet a standardized set of competencies established by Piedmont Health SeniorCare and approved by CMS before working independently.
  • Must only act within the scope of his/her authority to practice.

Education:Graduate from an accredited School of Nursing required. Masters Degree in Nursing preferred.

Required:At least 2 years supervisory experience.

Preferred:At least 5 years clinical nursing experience. Preferred experience in home care long term care setting and ideally, PACE experience. One year of experience with the frail or elderly preferred.

Current/valid License:RN currently licensed in NC; Presents credentials which demonstrate ability to meet performance criteria for the RN position; Valid NC drivers license.

Pay Range - $71,088 - $95,536
