Rutgers University Behavioral Health Care (UBHC), established in 1971, offers a full continuum of evidence based behavioral health and addiction services for children, adolescents, adults, and seniors throughout New Jersey. UBHC's 1,060 experienced behavioral health professionals and support staff are dedicated to treatment, prevention, and education. UBHC, one of the largest providers of behavioral health care in the country, has a budget of $260 million and has 15 sites throughout New Jersey. Services are readily accessible and include: inpatient, outpatient, partial hospitalization, screening, crisis stabilization, family/caregiver support, community outreach and case management, supportive housing, supported employment, prevention and consultation, employee assistance programs, and a licensed therapeutic school from preschool through high school.Specialty services include the New Jersey suicide prevention helpline and peer help lines for police, veterans, active military, teachers, mothers of special needs children and child protective service workers. In FY2016, UBHC treated 16,199 consumers, had 24,502 admissions, and touched the lives of 19,441 individual callers through peer support. In addition, UBHC is the primary mental health training resource for the New Jersey departments of Human Services, Children and Families, and Corrections, delivering 16,000 trainings each year.
Posting Summary
Rutgers The State University is seeking a Habilitation Counselor in the Intensive Recovery Treatment Services (IRTS) Statewide within Rutgers University Behavioral Healthcare.As a member of the Intensive Recovery Treatment Services (IRTS) team, the Habilitation Counselor will provide case management services for State prison incarcerated individuals with Opioid Use Disorders (OUD) to assist with transition from prison to the community.Among the key duties of this position are the following:
  • Provides case management services to assist individuals to gain access to needed medical, social, education, mental health, addiction, cooccurring and other services.
  • Visits individuals while incarcerated to assess and plan transitional services (Pre-Release).
  • Visits individual in the community and their homes throughout the state as per schedule developed with supervisor (Post-Release).
  • In accordance with unit requirements, consistently meets the current established unit face to face contact expectation with individuals served.
  • Develops a comprehensive assessment that addresses life domains such as housing, finances, transportation, legal services, vocational, employment, primary and specialized health care, and family strengths/needs.
  • Promotes the individual's recovery to his/her fullest potential by facilitating assessment of skills, including literacy, educational opportunities (GED, certification programs), job readiness skills, vocational training, employment placement and retention and career development.
  • Develops an Individualized Recovery Plan that is person-centered and includes strategies for recovery.
  • Identifies priorities, desired outcomes and the strategies and resources to be used in obtaining outcomes based on the case management assessment.

Qualifications

Minimum Education and Experience
  • Bachelor's Degree in the mental health field or other appropriate human services related field plus one (1) year of related experience.
  • Equivalent education, experience and/or training may be substituted for the degree requirement.
Certifications/Licenses
  • Applicants possessing a degree in Social Work must have an active New Jersey certification/licensure as required by the State Board of Social Work.
  • Valid driver's license required.
Required Knowledge, Skills, and Abilities
  • Must possess own vehicle or alternate means of transportation for use to outreach individuals served.
  • May be scheduled to work one (1) weekend day and/or occasional evenings.
  • Must be approved by the Department of Corrections (DOC) and adhere to all DOC requirements.
  • Employees will be subject to and must comply with all security regulations and procedures of NJ Department of Corrections and the assigned facility(ies) including, but not limited to sign in logs or electronic time clocks, searches of person and property, etc.
  • Violations of NJ Department of Corrections and/or being banned by the Department of Corrections to work at a NJ Department of Corrections' facility are grounds for a disciplinary action and potential termination.
Preferred Qualifications
  • Experience with NJ prison system and community based service provision to the adults with Opioid Addictions preferred.

Physical Demands and Work Environment
PHYSICAL DEMANDS:
  • This position requires visits to individuals incarcerated in the thirteen state prisons throughout the state in addition to community based outreach.
  • Individual wellness checks and crisis intervention required.
  • Subject to unpredictable situations to include behavioral, medical and psychiatric.
  • Occasionally exposed to patients exhibiting assaultive behaviors.
  • Exposure to blood borne pathogens that requires use of personal protective equipment.

WORK ENVIRONMENT:
  • Ability to speak clearly and write proficiently.
  • Hearing is essential.
  • The ability to focus on the task at hand despite numerous interruptions.
  • Must be able to sit for extended periods of time and document into an electronic medical record while in the field.
  • The employee spends much time driving, walking, standing, bending and talking with individuals served in the community located throughout the state.
  • The employee can expect to lift up to 30 pounds.
