POSITION SUMMARY
The Senior HIV/PrEP Navigator position will ensure patient and community engagement through outreach and care coordination of patients living with or at risk for HIV. This position will improve community awareness, patient care outcomes, and quality of life for this focused population. The Senior HIV/PrEP Navigator provides confidential education as well as facilitates testing and linkage to care to current Adelante patients and community members through external partnerships and events. The Senior HIV/ PrEP Navigator coordinates with clinical team members to ensure that effective patient care services are provided. This position navigates patients through the patient care delivery system, addresses social drivers of health, coordinates patient care services, and advocates for whole person health care. The Senior HIV/Prep Navigator is also able to carry out standing orders and submit prior authorizations as needed on behalf of the prescribing clinicians to expedite care. This role also provides education and support to staff to achieve program and organizational goal.

Qualifications
ESSENTIAL SKILLS AND EXPERIENCE:
High School Diploma or GED from and accredited institution
Medical Assistant Diploma or Certification from an accredited institution
Community Health Worker Certification preferred (or within 18 months of hire)
Three (3) years of working experience in community outreach setting or clinical setting.
Two (2) years of program coordination, tracking, and compliance experience
Bilingual (English/Spanish) required.
Demonstrates cultural competency and ability to relate to people living with HIV/AIDS, gay/bisexual men, trans individuals, as well as individuals from diverse socio-cultural and socioeconomic backgrounds.
Knowledge and experience with the Hispanic/Latino, Black/African American communities as well as the LGBT community.
Ability to speak with broad cross sections of the community about personal sexual behaviors, disease prevention, and drug use in a non-judgmental and caring way.
One (1) year of work and/or volunteer experience providing direct service to individuals from the LGBT community or individuals who are at high risk of contracting HIV or STIs.
Thinks and operates proactively in engaging a wider population in education for PrEP/PEP and HIV testing.
Experience with group facilitation, teaching and/or public presentations highly desirable.
Ability explain rapid HIV tests in compliance with Manufacturer’s instructions (experience preferred)
Ability to carry out standing orders and complete prior authorizations as appropriate and within scope of a medical assistant
Exceptional organizational skills are crucial to the quality and improvement of the program.
Basic understanding on how HIV is transmitted and prevented
Certification to perform cardiopulmonary Resuscitation for the Health Care Professional (CPR) and AED through courses that follow the guidelines from the American Heart Association and Red Cross (cognitive and skills evaluations)
Valid Level One Fingerprint Clearance Card issued by the Arizona Department of Public Safety for all specialty behavioral health locations
Physical Requirements:
Sitting, standing, and walking throughout the day for short to medium periods of time.
Occasional lifting of 20-50 pounds
Ability to set up tents, tables, chairs, and all supplies required at events
POSITION REPONSIBILITIES
- Manage navigator team’s outreach calendar
Assist new navigators with training and provide shadowing opportunities
Locate new and novel outreach locations and events by researching the wider community and building partnership with other organizations
Manage Inventory and organize marketing materials.
Must have reliable transportation in order to commute to outreach events educational and training presentations.
Must have flexibility in schedule to accommodate outreach, educational, and training presentations or events. This includes but is not limited to weekends and evening shifts.
Teach HIV/PrEP classes as appropriate within the scope of a CHW at community events and as needed to Adelante staff across all locations.
Actively promotes HIV Education, Testing, and Prevention Services through face to face and telephone contact with patients, community members, and external organizations
Perform duties, tasks, and assignments supporting the delivery of quality comprehensive case management and patient care within established time frames.
Maintain patient confidentiality by controlling the information being disclosed to authorized individuals ensuring compliance with all HIPAA and corporate compliance standards, as well as generally accepted confidentiality standards.
Accept all new referrals for the assigned program, completing the medical care coordination and navigation for all eligible patients.
Facilitates referrals for clinically indicated services outside the primary care office.
Coordinate a collaborative and well-shaped intake service resulting in effective utilization and quality medical health services.
Tracks patient follow-up and health outcomes.
Document all in-person and telephone encounters.
Maintains regular contact with enrolled clients to monitor progress towards treatment goals.
Connects patient with care team if concerns are noted due to changes clinical symptoms and treatment side effects or complications. .
Serves as a liaison and advocate for the patient and family, ensuring the delivery of quality and timely services to the patient from the patient care team and/or supporting government and community agencies.
Provides patient education within the scope of a CHW about available medical treatment options.
Develops and maintains positive and effective working relationships with patients, family members and clinical staff, as well as administrative and support team members to foster professionalism and coordination with the patient care team.
