Job Requirements:

To be considered for this opportunity, you must have the following:

• Active, unencumbered RN license within state of practice.

• 5+ years of experience as a registered nurse within primary care.

• Prior experience as a RN within acute care.

• Prior experience working with geriatric population.

Job Description:

• Manages daily nursing duties, including patient care activities, phone and in-person triage, and providing patient education.

• Works closely with Providers and the Center Manager to identify opportunities for enhancing the clinical performance of back-office staff and operations.

• Responsible for adhering to the clinical team workflows.
