UnitedHealth Group Hospice Liaison I - Remote in PST or MST in Phoenix, Arizona

This a task-heavy position processing hospice referrals. Much of your communication will be via email, Teams, or directly within the EMR system. The EMR system will be where the referral work will be processed and is the primary tool in this role. You will be responsible for communicating with providers and other team members and you will not have direct contact with patients or their family members. You can anticipate being on the phone about 25% of the time.

The Hospice Liaison I acts as a liaison between the Landmark clinical team and hospice agencies while processing hospice referrals and ensuring a timely and seamless transition to hospice care.

This position is full time, Monday - Friday. Employees are required to have flexibility to work any of our 8-hour shift schedules during our normal business hours of 8:00am-5:00pm PST (9:00am-6:00pm MST). On an as needed basis you may be asked to provide support up to 7:00pm CST to provide additional coverage.

We offer 4 weeks of on-the-job training. The hours during training will be 8:00am-5:00pm PST (9:00am-6:00pm MST), Monday - Friday. Training will be conducted virtually from your home.

Primary Responsibilities:

  • Process all incoming hospice referrals from Landmark providers in assigned territory

  • Coordinate accurate and appropriate documentation to facilitate the hospice admission

  • Obtain updates on and complete pending hospice referrals within established timeframe to meet departmental goals

  • Communicate updates and pertinent information to referring provider and interdisciplinary team

  • Prioritize all work, requests, and activities for each referral

  • Document in real time in the electronic medical record (EMR) Ubiquity

  • Hospice monitoring of patients who admit to hospice care for status updates

  • Establish and maintain positive working relationships with hospice providers and Landmark clinicians

Required Qualifications:

  • High School Diploma / GED or equivalent work experience

  • Must be 18 years of age OR older

  • Ability to work closely with a variety of stakeholders.

  • Ability to triage and prioritize tasks.

  • Experience providing issue resolution to customers & providers

  • Knowledge of hospice Medicare rules and regulations

  • Knowledge of managed care environment

  • Experience navigating multiple computer systems

  • Experience with Microsoft Office (including Microsoft Teams, Microsoft Excel, Microsoft SharePoint)

Preferred Qualifications:

  • 1+ year of hospice experience

Telecommuting Requirements :

  • Reside within MST OR PST

  • Ability to keep all company sensitive documents secure (if applicable)

  • Required to have a dedicated work area established that is separated from other living areas and provides information privacy.

  • Must live in a location that can receive a UnitedHealth Group approved high-speed internet connection or leverage an existing high-speed internet service.

California, Colorado, Nevada, or Washington Residents Only: The hourly range for this is $19.47 - $38.08 per hour. Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. UnitedHealth Group complies with all minimum wage laws as applicable.
