Position Description:
The Kidney-Pancreas Transplant Program cares for patients and their families across the continuum of advanced organ failure and transplantation. Our surgeons perform more than 200 kidney transplants each year making them one of the most comprehensive, high volume centers in the country for patients with end-stage renal disease. In May of 2013, The Penn Transplant Institute became the 14th institution in the country and the first in the region to perform 5,000 kidney transplants. Our program has been awarded a prestigious Silver Level Award by the U.S. Department of Health and Human Services (HHS) for our outstanding outcomes. Our Kidney-Pancreas Transplant Team is committed to advancing our vision of collaborative patient care, education of multidisciplinary trainees, and clinical research.
The Transplant Coordinator II is a professional nurse who safely practices with the UPHS professional practice model and demonstrates UPHS core values that reflect its mission. The Transplant Coordinator II provides direct care and coordinates care for patients with end-stage organ failure referred to the Transplant Program. The patients range in age from young adult to elderly. The Transplant Coordinator II demonstrates competency in patient and customer care, age-specific accountabilities, patient/employee safety accountabilities, operations, and core values.
In this role, the Transplant Coordinator II will provide patient care throughout the referral, evaluation, waitlist, and post-transplant phases of the transplant process. The Transplant Coordinator II assesses and evaluates patients and families developing a care plan in collaboration with the transplant team. The Transplant Coordinator II creates a written, continuous record for each patient; implements identified medical and nursing interventions; evaluates outcomes. The Transplant Coordinator II serves as an integral member of the transplant team in meeting the transplant patient's needs and ensuring continuity of care throughout the transplant process.
The Transplant Coordinator II maintains competencies for transplant patients' care and demonstrates the skills and knowledge to provide ongoing, individualized care. The coordinator complies with all regulations and standards of regulatory and accrediting bodies.
Minimum Requirements:
Minimum Certifications, Registry and/or Licenses Required:
-Licensed to practice professional nursing in the Commonwealth of Pennsylvania, R.N., and CPR certification.
Degree Requirements and Minimum Experience Required:
-Graduate of an NLN approved school of professional nursing. BSN required with four or more years of experience as a professional nurse.
Additional Information:
This position is a Monday through Friday position with rotational on-call duties.
Hours may be adjusted to meet the operational needs of the department.
Skill Requirements:
-Clinical assessment of adolescent, adult, and geriatric patients BCLS IV insertion Administration of medications (oral, IM, IV, PR) EKG Other nursing functions as determined Computers including windows, Microsoft Word, Microsoft Excel
