The abdominal solid-organ transplant program at Jefferson Einstein Philadelphia Hospital is part of the Jefferson Transplant Institute, and performs an average of 73 kidney/kidney-pancreas transplants and 66 liver transplants per year. Transplant Clinical Pharmacist Qualifications (required) middot; Graduate of ACPE accredited college of pharmacy (BS or PharmD) middot; Licensure to practice Pharmacy in the Commonwealth of Pennsylvania middot; PGY1 residency program completion or equivalent experience in pharmacy Qualifications (preferred) middot; PGY2 Transplant Pharmacy residency completion or equivalent experience in transplant pharmacy middot; Board certification in Solid Organ Transplantation Pharmacy (BCTXP) or board certification-eligible within 1 year of employment Job Description: Overview middot; The Transplant Clinical Pharmacist at Jefferson Einstein Philadelphia Hospital is responsible for providing comprehensive pharmacy services to abdominal solid organ transplant candidates, recipients and living donors. Responsibilities middot; Works as part of the multidisciplinary transplant team to assure optimal drug therapy and monitoring of transplant recipients and living donors on the inpatient transplant service. middot; Active participation in multidisciplinary rounds. middot; Ensures completion of thorough medication reconciliation. middot; Provides discharge planning and patient education. middot; Follows post-transplant recipients in the multidisciplinary transplant clinic. middot; Participation in transplant candidate and donor selection committee meetings. middot; Complete documentation as required by CMS. middot; Assists in the development/updating of existing practices, protocols, guidelines and order sets. middot; Serves as a drug information and educat onal resource for the pharmacy staff, medical staff, nursing staff and other healthcare professionals. middot; Precepts PGY1 Pharmacy residents and P4 Pharmacy students. middot; Opportunities to lead and participate in research and quality improvement projects. middot; Maintains education in transplant pharmacotherapy advances, guidelines, and literature to provide evidence-based care and contribute to the professional development of the transplant team. Scheduling Highlights middot; This is full-time, day shift position, Monday-Friday. There is no on-call or weekend staffing requirement.
