Summary:

  • The Living Donor LPN performs both clinical and administrative tasks and serves as a liaison for living donor patients within the Penn Transplant Institute (PTI). Makes observations, plans and organizes work, assesses patient needs under the supervision of a Transplant Coordinator or Physician. Provides patient care to living donor patients in the Penn Transplant Institute (PTI) based on the physical, psychosocial, cultural, educational, safety and related age criteria appropriate to the patient served in his/her assignment. Functions collaboratively within the team and is responsible for providing patient care as directed. Scope of work includes: facilitating referral process, scheduling coordination, obtaining medical records, faxing/emailing, confirming contact information, providing directions and appointment related specifics. Includes Epic documentation and other data collection platforms along with other duties as assigned.

Responsibilities:

  • Implements specific aspects of the plan of care as coordinated through the Transplant Coordinator or Physician

  • Performs technically oriented processes and assists with patient care requirements

  • Provides patient care that is within the scope of both professional and institutional standards

  • Obtains and tests specimens following standard operating procedures and documents results, taking appropriate action with unexpected results.

  • Collaborates with team members to deliver patient care that is appropriate to each patient. Communicates pertinent information to appropriate team members, within established guidelines

  • Utilizes electronic medical records and transplant related electronic platforms in accordance with established guidelines

  • Document all work performed in the electronic health record, and/or other data collection platform

  • Participates in development and providing appropriate patient and family education

  • Functions with awareness of fiscal and material resource management

  • Completes all mandatory education requirements

Credentials:

  • Licensed Practical Nurse (Required)

  • Basic Cardiac Life Support (Required)

Education or Equivalent Experience:

  • Other (Required)

  • Education Specialization:Graduate of an approved Practical Nursing programEquivalent Experience:•And 2+ years Acute care, ambulatory care, or long-term care experience as a licensed practical nurse
