Case Manager RNs are responsible for the following:

Advocates for the patient and family throughout the entire episode of care. Monitors patientrsquo;s progression intervening as needed to ensure efficient patient progress. Coordinates all aspects of discharge planning and establishes a safe discharge plan.

The Case Manager RN utilizes advanced nursing skills and knowledge to comprehensively coordinate care for a defined patient population in order to facilitate patients through the continuum of care.

Bachelors of Nursing required Three (3) + years case management/discharge planning experience CCM certification preferred Active RN license in NJ/PA
