Primary Responsibilities:

  1. Schedule all OBRA and PPS MDS assessments per RAI manual guidelines.
  2. Complete assigned observations to support OBRA MDS.
  3. Audit and review all admission for correct ICD 10 coding.
  4. Complete MDS assigned sections and CAA summaries (if applicable) following the OBRA and PPS schedule per RAI manual (Ch. 2).
  5. Complete all Discharge and Entry tracking forms per RAI manual guidelines.
  6. Complete corrections to MDS as needed.
  7. Complete and monitor all MDS transmissions.
  8. Coordinate Restorative Programs with resident and/or Therapy (if applicable).
  9. Schedule Care plan meetings with residents and/or resident representatives.
  10. Participate in Medicare Meetings.
  11. Facilitate Plan of Care Preview meeting.
  12. Monitor and review Case Mix Index.
  13. Monitor and review Quality Measures/5 star reports and CASPER reports.
  14. Updating 672 and 802-Matrix for Providers.
  15. Other duties as assigned.

Specific Duties:

  1. Schedule/assign ARD (assessment reference date) for all MDS assessments.

    1. OBRA (Admission, Quarterly, Annual, and Significant Change) .
    2. PPS (5 day and IPA)
  2. Complete/review all assigned observations to support OBRA MDS assessments.

    1. Pain Assessment for the Cognitively Aware OR Pain Assessment for Advanced Dementia (PAINAD).
    2. Braden Scale (first 3 weeks following the initial Admission/readmission Braden Scale and with OBRA schedule).
  3. Complete assigned MDS sections and CAA Summaries (if applicable).

    1. Refer to MDS section IDT assignments (NH 108).
  4. Audit and review all ICD 10 coding on admission. Select the appropriate Primary and Admission diagnoses based on medical condition.

  5. Complete Clinical sequencing for all Medicare A and Managed Care/Like Medicare A.

  1. Complete Corrections to the MDS as needed.

  2. Complete and Monitor MDS transmission process using Matrix Care and SimpleLTC.

    1. Review all validation code errors for accuracy, correct as needed.
    2. Refer to MDS coordinator training manual.
  3. Keep and monitor Skilled Case Mix Groups worksheet (NH 220) for all residents with the following payers: Medicare A, Medicare Replacement, Managed Care, MSP (Medicare Secondary Payer).

  4. Participate in Medicare Meeting per policy. (IL 1.39).

    1. Comprehensive Medicare meetings weekly.
    2. Assist with keeping the Comprehensive Medicare meeting worksheet updated.
    3. Collect ARD report from therapy.
  5. Facilitate Plan of Care Preview meeting.

    1. Schedule and communicate the Plan of Care Preview meeting (PCPM) with the IDT as outlined in the procedure.
  6. Monitor and review Case Mix Index in conjunction with Care Plan Coordinator.

    1. Run MDS Advisor report, MDS 3.0 Medicaid Case Mix Index report in Matrix Care quarterly based on snapshot dates (Jan 1- Mar 30, Apr 1-Jun 30, July 1- Sept 30, Oct 1-Dec 31).
    2. Review each resident CMI based on state specific RUG category requirements for accuracy.
  7. Monitor and review Quality Measures report and 5 star report in conjunction with Care Plan Coordinator.

    1. Monitor SimpleLTC for 5 star report monthly.
    2. Run facility level quality measure report and resident level quality measure report monthly.
    3. Distribute Quality Measure reports and 5 Star Report to Administrat r, Director of Nursing, and Assistant Director of Nursing monthly.
    4. Run Missing Assessment report quarterly and correct as needed.
  8. Updating 672 and 802-Matrix for Providers.

    1. Review and update survey readiness forms Quarterly.
    2. Review and update survey readiness forms weekly when in survey window.
  9. All other duties as assigned.
