Responsibilities

Job Summary:

A passionate and dedicated to connecting with our community, providing essential health information and resources, and fostering a culture of health and wellness. The position requires a self-starter, a strong communicator, and deeply committed to serving those in need.

Essential Functions:

  • Community Engagement

  • Develop and implement outreach strategies to engage with diverse community members, including those experiencing homelessness.

  • Build relationships with local organizations, community leaders, and residents.

  • Participate in community events and health fairs to raise awareness about health issues and resources.

  • Health Education and Information

  • Provide accurate and accessible health education on various topics, including basic nutrition, hygiene, chronic disease prevention, and mental health.

  • Distribute health information materials and resources.

  • Outreach Services for Individuals Experiencing Homelessness

  • Conduct outreach to individuals experiencing homelessness

  • Advocate for the needs of this population and build partnerships with relevant organizations.

  • Food Backpack Program

  • Manage and coordinate the distribution of food backpacks to children experiencing food insecurity.

  • Collaborate with schools and community partners to identify eligible families and ensure efficient delivery.

  • Track program data and report on outcomes.

  • Administrative Tasks:

  • Maintain accurate records of outreach activities, program participation, and resource utilization.

  • Prepare reports and presentations for internal and external stakeholders

Qualifications

Required Education and Experience:

  • High School diploma or equivalent

  • Minimum of two (2) years of experience in community engagement, health education, or social services

  • Experience working with individual’s experiencing homelessness

  • Bilingual proficiency (Spanish preferred)

  • Experience in program development and implementation

Required Licensure and/or Certification:

  • Current OR driver license with clean MVR

  • AHA Basic Life Support Certification

Required Minimum Knowledge, Skills and Abilities :

  • Strong communication and interpersonal skills, with the ability to build rapport with diverse populations

  • Passion for improving community health and wellness, particularly for vulnerable populations

  • Ability to work independently and as part of a team

Preferred Qualifications:

  • Bachelor’s Degree in Public Health, Social Work, Health Education, or a related field

  • Experience in program development and implementation

  • Knowledge of local community resources and social services

Pay Range

$25.18 - $36.52 /hour
