FlexStaff Nurse Case Manager/Residential RN in Patterson, New York

FlexStaff is seeking a Nurse Case Manager/Residential RN to join our client's team, a leading organization that provides housing, services and programs for individuals with intellectual and developmental disabilities.

The Nurse Case Manager will coordinate the delivery of health care services for persons receiving services residing in residential facilities. This position will cover various locations in the Hudson Valley primarily Putnam and Dutchess counties.

Responsibilities

  • Coordinates the delivery of routine and specialty health care services for adults with disabilities residing in residential programs

  • Assesses and establishes health care outcomes for persons receiving services. Updates assessments on at least a semi annual basis or as indicated. Maintains health care profiles.

  • Provides guidance and training to direct care staff to ensure proper delivery of health care services.

  • Develops plan of nursing services and oversees the implementation of the health care plan by educating person's receiving services, DSP/Caregivers and families in the plan of nursing services.

  • Evaluates the effectiveness of preventive/ongoing health care and makes recommendations/ referrals for alternative care, as necessary.

  • Acts as a liaison to communicate health information between and among team members, families, other agencies, hospitals, clinics and all other service providers

  • Provides on call triage coverage via cell phone between the hours of 6:00 am - 6:00 pm, Monday thru Friday. Is prepared to go onsite if an on call problem warrants a nurse's visit.

  • Completes relevant medical documents, assists in developing policies, procedures and forms; and establishing working systems.

Requirements

  • NYS RN License, BSN preferred.

  • Prior nursing experience and/or experience working with persons with developmental disabilities preferred.

  • Excellent oral and written communication skills.

  • Able to work flexible hours and/or days.

The salary range for this position is $88,000-$92,000/year
