Guthrie Outreach Coordinator-LPN-Value Based Reimbursement-Full Time in PA, United States

Position Summary:

Dedicated to centralizing and coordinate health promotion and risk factor modification activities for the needs of population health. Responsible for creating, organizing, scheduling, and promoting outreach efforts as well as collaborating with clinical and financial leadership to support fulfillment of quality metrics. Complete patient outreach and assist with population health activities as assigned.

Education, License & Cert:

LPN with current license in State where employment is located.

Experience:

Minimum 3 years clinical experience. The outreach coordinator will have extensive communication with the patient population and will need the ability communicate effectively both verbally and in writing. Must have effective interpersonal and customer service skills, including professional phone etiquette with a positive, nurturing, and flexible attitude. Must be able to interact with all age, social and economic levels. Organize, prioritize, and manage time efficiently to meet established deadlines. Operate PC and other software as required to perform job duties such as preparing reports. Maintain effective interpersonal relations.

Essential Functions:

  1. Coordinate activities related to population health initiatives including but not limited to various patient outreach efforts.

  2. Analyze quality metrics and participate in efforts for population health management, including comprehension of patient records for correlation of appropriate metrics.

  3. Educate patients and provide information in a knowledgeable, friendly, and effective manner.

  4. Preparation of reports summarizing activities and performance metrics related to process improvement initiatives.

  5. Participate in process improvement activities as needed.

  6. Participates in problem resolution activities to help eliminate barriers to patient care.

  7. Participates in professional development activities and maintains professional affiliations. Maintains confidentiality.
