RESPONSIBILITIES:

To assist residents with/or provide all levels of activities of daily living (ADL's) and any other needs the resident may require consistent with the facility's policies and procedures.

FUNCTIONS:

  • Must be available to periodically work more than eight (8) hours per day, or more than forty ( 40) hours per week.

  • Assist residents with personal care including oral/denture hygiene, bathing (bed bath, Century Tub and/or shower), foot care, nail care, hair care, shampooing, shaving, grooming and dressing.

  • Transfer, ambulate and transport residents.

  • Provide active/passive ROM (range of motion) with daily care.

  • Assist residents with toileting needs.

  • Provide peri care and catheter care.

  • Maintains resident rooms in an orderly fashion

  • Serves meals when necessary, give nourishments and fluids, feeds or assists residents according to needs.

  • Checks vital signs as directed - vital signs include temperature, pulse, respiration and blood pressure. If a vital sign( s) is unusual, alert the nurse immediately.

  • Clean and maintain equipment and utility rooms as assigned.

  • Provide post-mortem care.

  • Obtains and records resident weights in CareTracker.

  • Completes documentation - including AOL monitoring, special device records, I&O, toileting schedule, intake studies and BM record - in a timely manner in CareTracker system.

  • Refers visitors and family members to Charge Nurse, Unit Manager and/or Nursing Supervisor for information regarding resident's condition.

  • Takes all breaks off floor. Always checks with Charge Nurse before leaving the floor for any reason.

  • Observes resident behavior and reports any changes or unusual fmdings to Charge Nurse.

  • Assists residents in preparing for doctor's visits, religious services, rehabilitation services, diagnostic services and social events.

  • Gives prompt and efficient assistance to the residents, answers call bells promptly.

  • Keeps incontinent residents clean and dry. Changes clothing and or linen as necessary. Give peri care to residents as required

  • Changes bed linen and makes beds in an appropriate and timely manner.

  • Turns and changes position of resident Lifts residents as needed

  • Attends 12 hours of in-services per year (6 per 6 months) and attends nursing meetings.

  • May accompany residents on outings and/or medical appointments.

  • Other related duties that may become necessary or as directed by the Supervisor, Department Head and/or Administrator

6:00 AM-10:00 AM/5:00 PM - 9:00 PM CNA Responsibilities:

  1. Report to your assigned unit. Report in with the Charge Nurse on duty and receive your assignment2. Complete assignment that may encompass responsibilities as outlined on pages 1 and 2.3. Insure that resident rooms are left in an orderly fashion Complete all required paperwork:.4. Report off to the Charge Nurse and the CNA assuming your assignment prior to leaving the unit

QUALIFICATIONS:

  • Completion of a 100-Hour Nurse Aide Training Program

  • Completion of New York State Residential Health Care Facility Nurse Aide clinical skills and written exam and PROOF of certification. If certified in another state, must have obtained New York State certification via reciprocity.

  • Previous experience preferred

  • Sense of responsibility to job/residents, moral integrity and understanding is required

  • Ability to work cooperatively with others as part of a team effort

  • Good mental and physical health required

  • Sincere desire to work with the aged

  • Pleasant, cheerful, tactful and cautious with residents, relatives and visitors also other staff members

  • Flexibility and ability to remain calm in emergencies

  • Treat resident information confidentially

  • Knowledge of resident's rights

  • Existing employees must have a satisfactory work record in their prior positions
