GENERAL SUMMARY AND PURPOSE:

Provides advocacy, education, assessments and interventions to patients and families in the inpatient hospital setting to achieve an optimal level of care. Coordinates with medical staff and outside organizations to establish specific treatment plans and facilitate transition planning to achieve quality clinical outcomes.

SKILLS, KNOWLEDGE, EDUCATION AND EXPERIENCE :

  • Master Social Worker (LMSW) required as defined by their primary work state (Idaho or Oregon), Licensed Clinical Social Worker (LCSW) preferred. * All colleagues must provide licensure or proof of application in process for the secondary state within 6 months from hire date.

  • Master’s Degree in Social Work required.

  • Two years of experience in hospital setting or medical social work setting preferred.

  • Experience in case management, addiction services, mental health, or population health management desired.

ESSENTIAL FUNCTIONS :

  • Knows, understands, incorporates, and demonstrates the Organization's Mission, Vision, and Values in behaviors, practices, and decisions.

  • Provides interventions for patients, families, and caregivers as appropriate. Interventions may include but are not limited to brief counseling, supportive therapy, crisis intervention, grief counseling, psychiatric placement/referral, alcohol/drug assessment and referral, and referral to alternative community resources.

  • Participates in care coordination activities for those patients identified as needing social work and/or complex case management interventions.

  • Works collaboratively with healthcare team members to efficiently allocate resources.

  • Accurately documents assessments, plans, and interventions in the EMR.

  • Responsible for coordination of care to mental health and/or substance abuse facilities.

  • Responsible for following established patient choice policy.

  • Responsible for coordinating with physicians and other legal entities relating to, but not limited to, guardianships, involuntary psychiatric holds, power of attorney, and advanced directives.

  • Collaborates with physicians and hospital staff through formal and/or informal multidisciplinary case conferences to meet the emotional, mental, physical and spiritual needs of patients and/or their families/caregivers

  • Responsible for weekend and holiday social work coverage.

  • Actively participates in department staff meetings and other teams or committees as required

  • Demonstrates knowledge and skills to competently care for all assigned age groups (Neonate, Child, Adolescent, Adult, Geriatric as applicable).

  • Adheres to the National association of Social Work (NASW) code of ethics and the State Licensure Board of ethics.
