Adecco Healthcare is searching for a Social Worker (LMSW) to become part of our client's team in Omaha, NE . This is a direct-hire, full-time, and permanent opportunity.
Will be helping patients for the 3 Omaha, NE locations for the client.
Pay
$60,000-$80,000 (depending on experience)
Qualifications - Education & Experience
Required:
LMSW license in NE.
Master's Degree in Social Work.
At least 1 year of experience as a clinical Social Worker.
Experience with screening, assessing, and planning for common social services needs.
Knowledge of different diagnoses of common mental health conditions.
Strong interpersonal communication skills.
Preferred:
Proficient PC skills.
Bilingual in Spanish or other languages spoken in the community.

Job Summary
Build relationships and collaborate with primary care teams to perform social services assessments and identify the specific needs of each member.
Plan, coordinate, manage, and implement support packages to assist members in overcoming socioeconomic and medical challenges.
Navigate managed care plans to access community services and programs.
Provide case management for social and behavioral care, enabling members to self-manage their health and social service support.
Ensure accurate and timely documentation of patient encounters and sessions in all clinical management systems.
Actively seek ways to enhance the company's approach based on feedback and regularly conducted surveys.
Support members and their families during advance care transitions.
Assist members in accessing state-based prescription programs and other benefits.
