Liberty Healthcare Corporation Incident Analyst – health and human services in OK, United States

A full-time Incident Analyst position is currently available with Liberty Healthcare at the Robert M. Greer Center in Enid, OK.

The Greer Center is a 52-bed ICF/IID which provides specialized short-term residential treatment services to adults who have intellectual disabilities and behavioral support needs.

The Greer Center endeavors to help each individual build lifelong skills so they may achieve and maintain good health, well-being, and independence in the community.

In this role you will contribute to a culture of quality and help ensure outstanding care is provided to the people we serve.

Your day-to-day responsibilities will include:

  • Monitoring and ensuring compliance with incident reporting to regulatory bodies

  • Reviewing client incident reports for completion and accuracy

  • Ensuring all required documentation has been submitted by appropriate parties

  • Tracking and trending incident reports and behaviors related to a risk management threshold algorithm

  • Monitoring the risk management threshold tracker to ensure all clients at risk will be monitored and identified

  • Notifying appropriate parties to organize a team meeting to implement a plan to reduce the client’s risk

  • Following up with the appropriate parties to ensure the meeting occurs on a timely basis

  • Maintaining the risk management tracking tool and initiating alerts to the management team

  • Assisting in monitoring and tracking the form submittals to the licensing and regulatory entities

  • Assisting in the monitoring and completion of all investigations

  • Participating in the daily incident review meetings

  • Providing data and reports to management

  • Tracking and trending the outcomes of the action plans for clients on the high risk threshold list

  • Participating in the performance improvement, risk management and utilization review committee

  • Assisting in data tracking, monitoring and reporting high-risk matters

As a full-time employee of Liberty in this role, you can expect:

  • $26.34 per hour (approximately $54,700 annually)

To qualify for this position, you must possess the following qualifications:

  • Knowledge of Title XIX, CMS, and relevant State of Oklahoma and Federal rules, standards, and regulations

  • Experience in quality assurance, risk management, and auditing

  • Experience in the intellectual and developmental disabilities field - or related health and human services experience

  • Ability to develop quality improvement plans, data sets, and reports

  • Solid computer skills

  • Ability to develop and maintain collaborative relationships with a wide array of individuals

A Bachelor’s degree in a human services or a related field is preferred, however relevant work experience will be considered.
