Population Health Specialist

Position Summary:

The Population Health Specialist is responsible for engaging providers and their staff through a data-driven approach to implement and optimize value-based care workflows to improve population health management, including cost, quality, access, network optimization, and documentation and coding. The Population Health Specialist will assist the Population Health Manager and work directly with assigned provider organization(s) to achieve outcomes across provider engagement initiatives. They will work to positively influence Accountable Care Team behavior and support practices in achieving value-based care, quality, and cost & utilization goals. This person will play a critical role in supporting the collaborative relationship we seek to establish at the practice or clinic level by providing value-add services to our provider partners.

Job Description:

The Population Health Specialist will be a blend of remote and on-site support at our client's site in Cincinnati, OH. The PHS will need to accommodate on-site engagements as needed which will range from 3-5 days/week.

Role and Responsibilities

  • Support Population Health Manager in successful implementation of provider engagement initiatives or programs at a practice or clinic level

  • Contribute to improved provider experience with transition to value-based care and population health management through practice-based support

  • Provide coaching and management on pre-visit planning techniques and closure of gaps in care at the practice or clinic level

  • Actively participate in workflows to support gap closure and other targeted provider engagement initiatives

  • Follow practice progress, identify potential concerns, and collaborate with Population Health Manager to proactively manage observed opportunities

  • Ensure that expectations of members, providers, and medical management staff are continuously met, and problems are presented and addressed as they occur for immediate resolution

  • Establish strong personal relationships with Accountable Care Teams (providers, clinical staff, administrative staff and management)

  • Stay abreast of current industry, market, and policy landscape

  • Perform other duties as assigned

Experience, Education and Qualifications

  • Bachelor’s degree in Healthcare Administration, Business Administration preferred or related field

  • Minimum of 2 years of healthcare experience; medical assistant or another practice-based role, patient navigation, or related role

  • Ability to operate in a fast-paced and dynamic environment

  • Team player, excellent communication skills

  • Organized – detail-oriented; consistently meets and tracks timelines and objectives.

  • Knowledge of the U.S. healthcare system

The hiring range for this position is:

$49,200.00-$63,000.00
